Background and objective:Helicobacter pylori(HP)infects more than 50%of the world’s population and is the main causative factor for gastric cancer,but it is also associated with the occurrence and development of many extragastric diseases.The relationship between HP infection and its related gastritis and colorectal polyps and colorectal cancer has received extensive attention,but the views given by domestic and foreign scholars are inconsistent.This study mainly explores the risk factors of colorectal polyps,the relationship between HP infection and its related gastritis and colorectal polyps,and explores the relationship between the characteristics of colorectal adenoma patients and HP infection,so as to provide a basis for the prevention and treatment of colorectal polyps and adenomas.Methods:1.Select adult inpatients who came to our hospital for colonoscopy from May 2017 to March 2019.A total of 918 subjects were included through the inclusion and exclusion criteria,including 686 in the polyp group,232 in the non-polyp group(control group),622 with adenomatous polyps,and 64 with non-adenomatous polyps,of which gastroscope and gastric mucosal pathological examination were performed.450 cases.2.To compare the age,gender,HP infection,smoking history,drinking history,hyperlipidemia,diabetes history,family history of colorectal cancer,CRP,PCT,CEA,CA199,CA125 between the polyp group and the polyp-free group.Univariate and multivariate analysis to find risk factors for colorectal polyps.3.To compare the difference in the infection rate of Helicobacter pylori between different pathological types of colorectal polyps and the control group.4.According to the number,growth site,size,pathological type,and tissue grade of adenomatous polyps,they were divided into subgroups,and the differences in the HP infection rate among the subgroups were compared.5.450 patients who underwent gastroscopy and gastric mucosal pathological examination were divided into non-atrophic gastritis,atrophic gastritis,gastric mucosal intestinal metaplasia,gastric mucosal dysplasia with non-atrophic gastritis as the control group to compare the incidence of colorectal polyps among different gastric mucosal lesions.6.According to HP infection,all patients undergoing gastroscopy were divided into HP(-)non-atrophic gastritis,HP(+)atrophic gastritis,HP(+)gastric mucosal intestinal metaplasia,HP(+)gastric mucosal dysplasia in four groups.The HP(-)non-atrophic gastritis was used as the control group,and the incidence of colorectal polyps was compared with the other three groups.Kendall’s tau-b correlation evaluation was used to study the correlation between HP-related gastritis and polyps.Results:1.Univariate and binary logistic regression analysis showed that age(OR=1.075,95%CI 1.057~1.094,P<0.001),male(OR=2.615,95%CI 1.794~3.811,P<0.001),HP Infection(OR=2.610,95%CI 1.838-3.707,P<0.001),smoking history(OR=2.116,95%CI 1.267~3.532,P=0.004),hyperlipidemia(OR=2.100,95%CI 1.474-2.992,P<0.001)and family history of colorectal cancer(OR=15.865,95%CI 1.938-129.895,P=0.01)were the risk factors for colorectal polyps.2.The infection rate of HP in the non-adenoma polyp group and the adenoma polyp group was higher than that in the non-polyp group,and the difference was statistically significant(P<0.001).3.There was no significant difference in the number of adenomas,adenoma size,adenoma growth site,adenoma pathological type and adenoma tissue grade in the HP infection rate of adenomatous polyps(P>0.05).4.The incidence of colorectal polyps in patients with atrophic gastritis,gastrointestinal metaplasia and gastric dysplasia was higher than that in the non-atrophic gastritis group,and the differences were statistically significant(P<0.05).5.The incidence rates of HP(+)atrophic gastritis,HP(+)gastric mucosal intestinal metaplasia,HP(+)gastric mucosal dysplasia non-adenomatous polyps and adenomatous polyps were higher than those of HP(-)non-atrophic gastritis,the difference was statistically significant(P<0.05).There was a positive correlation between HP-related gastritis and colorectal polyps,Kendall’s tau-b=0.481,P<0.001.Conclusions:1.HP infection,age,smoking,male,hyperlipidemia,and family history of colorectal cancer are independent risk factors for colorectal polyps.2.HP infection is associated with the occurrence of non-adenomatous polyps and adenomatous polyps,but HP infection was not associated with the number,size,growth site,pathological type,and tissue grade of adenomas.3.Different gastric mucosal lesions are related to the occurrence of colorectal polyps,and HP-related atrophic gastritis,HP-related gastric mucosal intestinal metaplasia,HP-related gastric mucosal dysplasia are positively correlated with the occurrence of colorectal polyps and adenomas.
