| Objective:To observe the clinical efficacy of Bushen Zhuangjin Decoction combined with PFNA in the treatment of elderly femoral intertrochanteric fractures,and to explore its effectiveness and clinical advantages,so as to provide new ideas and methods for the clinical treatment of elderly femoral intertrochanteric fractures.Methods:Selected from October 2019 to October 2020,106 elderly inpatients with intertrochanteric fracture diagnosed in the Department of orthopedics and traumatology,people’s Hospital Affiliated to Fujian University of traditional Chinese medicine were randomly divided into control group and experimental group,53 cases in each group.Control group: PFNA +conventional anti osteoporosis treatment(calcium carbonate D3 particles + calcitriol capsules);experimental group: on the basis of the treatment method of the control group,Bushen Zhuangjin decoction was added;both groups received functional exercise after operation.VAS score,swelling subside,Harris score,X-ray score of callus healing,TCM symptom score and curative effect evaluation were observed before operation,2 weeks and 3 months after operation.Results:(1)There was no significant difference in age,gender,course of disease,VAS score,swelling,Harris score,fracture site and fracture classification between the two groups,which was comparable(P> 0.05).(2)VAS score: Two groups of patients after 2 weeks and preoperative comparison,VAS score decreased,the difference was statistically significant(P < 0.05),postoperative 3 months and postoperative 2 weeks,VAS score was lower,the difference was statistically significant(P <0.05);postoperative 2 weeks,postoperative 3 months,VAS score in the experimental group were lower than the control group,the difference was statistically significant(P < 0.05).(3)Swelling: Two groups of patients after 2 weeks and before operation,the swelling value decreased,the difference was statistically significant(P < 0.05),3 months after operation,compared with 2 weeks after operation,before operation,the swelling value was lower,the difference was statistically significant(P < 0.05);2 weeks after operation,the swelling value of the experimental group was lower than that of the control group,the difference was statistically significant(P < 0.05);3 months after operation,the swelling value of the experimental group was higher than that of the control group,the difference was statistically significant(P < 0.05),There was no significant difference in swelling value between the two groups(P > 0.05).(4)Harris score: Two groups of patients in 2 weeks after operation compared with preoperative,Harris score increased,the difference was statistically significant(P < 0.05),3months after operation compared with 2 weeks after operation,preoperative,Harris score was higher,the difference was statistically significant(P < 0.05);2 weeks after operation,3months after operation,Harris score of experimental group was higher than that of control group,the difference was statistically significant(P < 0.05)).(5)Comparison of X-ray score of callus healing: Comparison of X-ray score of callus healing before and after treatment: the X-ray score of callus healing in the two groups increased at 1month after operation compared with that before operation,and the difference was statistically significant(P < 0.05).The X-ray score of callus healing was higher at 3 months after operation compared with that at 1 month after operation and before operation,and the difference was statistically significant(P < 0.05),The X-ray score of callus healing in the experimental group was higher than that in the control group,and the difference was statistically significant(P < 0.05).(6)Comparison of TCM symptom scores:Three months after operation,the total score of TCM symptoms of the two groups were lower than that before operation,the difference was statistically significant(P < 0.05);there was no statistically significant difference in the total score of TCM symptoms of the two groups before operation(P > 0.05),the total score of TCM symptoms of the experimental group was significantly lower than that of the symptomatic group,the difference was statistically significant(P < 0.05).(7)Curative effect comparison: two groups of patients after treatment,the experimental group was significantly higher than the control group,the difference was statistically significant(P<0.05).Conclusion:the application of Bushen Zhuangjin decoction combined with PFNA in the treatment of senile intertrochanteric fracture has good clinical effect,which can effectively reduce postoperative hip pain,promote hip swelling subside in a short time,promote callus growth and improve joint function,and improve the therapeutic effect. |
