Abstract/Summary:
Objective:Measure the organ absorbed dose(Dr),the Entrance Skin Dose(ESD),the Peak Skin Dose(PSD)in and out of the irradiation field during different interventional procedures,and estimate the effective dose.Analyze radiation dosimetry problems.Provide a basis of interventional staff to better control the exposure to interventional surgery,with regard to protection issues,provide some guidance.Method:Place the radiochromic EBT3 films on the anthropomorphicphantom incident body surface.Thermoluminescence dosimeter(TLD)components are placed in the reserved cavity of the anthropomorphic phantom.Simulation of Male Cardiac Interventional Surgery Using Digital Subtraction Angiography System use digital subtraction angiography system to simulate male interventional cardiac surgery,female head angiography,female interventional cardiac surgery(including small irradiation field mode),and uterine embolization,and all are irradiated from the back of the human body to the front.Collect film and TLD components after irradiation,measure the exposure dose for each section,and analyze the organ absorbed dose,ESD,PSD in different surgical types.Results:1.The highest absorbed dose of each organ in head angiography was 96.776 mSv in the brain,and the lowest in the bladder were 0.046 mSv;ED is 6.071 mSv; ESD is 193.4 m Gy;PSD is 370.5 m Gy;Theorgan absorbed dose and Dose-area product(DAP)conversion factor(Dr/DAP)is between 0.001 mSv·Gy-1·cm-2and 1.721 mSv·Gy-1·cm-2;PSD and DAP conversion factor(PSD/DAP)is 6.589(10-3·cm-2);The dose index(ESD/CD)is 0.713.2.The highest organ absorbed dose in female cardiac interventional surgery was the adrenal gland,which was 95.694 mSv in the conventional irradiation field mode and 86.614 mSv in the small irradiation field mode;The absorbed dose to the eye body is the smallest at 0.178 mSv in the conventional irradiation field mode and 0.143 mSv in the small irradiation field mode;ED is 30.812 mSv and26.846mSvrespectively;ESD is 151.5m Gy and 180.5m Gy respectively;PSD is 221.8m Gy and 280.8m Gy respectively;The organ absorbed dose and DAP conversion factor in conventional irradiation field mode are between 0.005mSv·Gy-1·cm-2and 2.528 mSv·Gy-1·cm-2;Small irradiation field mode is between 0.004 mSv·Gy-1·cm-2to 2.793 mSv·Gy-1·cm-2;The conversion coefficients of PSD and DAP are 6.412(10-3·cm-2)and 10.396(10-3·cm-2);The dose indices are 0.899 and 0.612,respectively.3.The ESD in the conventional irradiation field of male cardiac interventional surgery was 177.1m Gy;PSD is 249.2m Gy;The conversion factor between PSD and DAP is 5.793(10-3·cm-2);The dose index is 0.847.4.The highest organ absorbed dose in uterine embolization is 136.349 mSv in colon;The absorbed dose to the eye body is the smallest at 0.045 mSv;ED is 33.058mSv;ESD is 208.4 m Gy;PSD is 284.6 m Gy;Theorgan absorbed dose and Dose-area product(DAP)conversion factor(Dr/DAP)is between 0.0004mSv·Gy-1·cm-2and 1.449 mSv·Gy-1·cm-2;PSD and DAP conversion factor (PSD/DAP)is3.023(10-3·cm-2);The dose index(ESD/CD)is 0.455.Conclusions:Reducing the irradiation field can reduce the dose of unnecessary irradiation organs to a certain extent,but we can’t blindly choose small irradiation field to implement the diagnosis and treatment procedures,try to reduce the effective dose of the whole body,and pay attention to the protection of the incident skin at the same time.In cardiac interventional surgery and uterine embolization,PSD is mainly distributed near the spine,and the proportion of red bone marrow in the spine is relatively high.It is suggested to pay attention to the protection of the spine in the process of interventional diagnosis and treatment to reduce the irradiation dose of red bone marrow.The ESD and PSD of patients with larger size are correspondingly higher,so doctors should pay attention to the patient dose when performing surgery on patients with larger size.This experiment obtains the conversion coefficient between DAP and Dr,ED and PSD of different surgical types.The CD value displayed by the ionization chamber of the DSA equipment used this time overestimates the actual ESD value.When estimating the patient’s dose,comprehensive evaluation should be carried out from many aspects to ensure a more accurate understanding of the patient’s radiation dose.
