Objective1.It is proposed to confirm the therapeutic effect of moxibustion on chronic atrophic gastritis;2.It is proposed that moxibustion may control the expression of related genes by affecting the PTEN/PI3K/Akt signaling pathway,and effectively control the gastric mucosal injury of CAG to achieve the purpose of treating CAG and preventing precancerous lesions.Methods Male Wistar rats were randomly divided into normal groups,and the modules were made.The N-methyl-N-nitro-N-nitrosoguanidine(MNNG)inducer was combined with the tail stimulation and hunger-smelling method.CAG rat model.After34 week,CAG model has successfully identified,CAG rats are divided into 3 groups,a model group,a herbal cake-separated moxibustion group,and a western medicine group.The normal group and the model group were given fixed bundling;the herbal cake-separated moxibustion group took Zhongwan and Qihai points to separate the medicine moxibustion 2 zhuang,once a day,6 times a week for 4 weeks;the western medicine group was given folic acid suspension 1.0 Mol/100g(ie folic acid 2mg/kg)was administered intragastrically;once a day,6 times a week for 4 weeks.From the general condition of rats and the morphological changes of gastric antrum,the effect of blocking ginger and moxibustion on CAG was observed.Observing the autophago-somes of gastric antrum in each group by transmission electron microscopy.RT-q PCR and immunohistochemistry were used.The expression levels of PTEN,PI3 KCa,caspase-9and P53 and the expressions of PTEN,p-akt,caspase9,PIP2 and MDM2 in gastric antrum of each group were detected.Results(1)Comparison of the general conditions of the rats in each group: Compared with the normal group,the CAG rats were easily yellowed and dry,dull and dull,with signs of atrophy,two eyes and less gods,pale auricles,pale and slow response.Do not like activities,easy to panic,like to pile up in the corner of the squirrel cage,less diet,soft stool,no shape or dry stool,the smell of the rodent cage stinks;after the intervention of the cake group and the western medicine group,the hair loss and gloss The model group improved and the spirit improved.The auricle gradually changed from pale to reddish,the weight and activity increased,the eating increased slightly,and the feces gradually formed.(2)Comparison of body weight of rats in each group: After 34 weeks of modeling,compared with the normal group,the weight loss of the CAG model group was statistically significant(P<0.05).After 4 weeks of treatment,compared with the CAG model group,the weight gain of the separate cake moxibustion group and western medicine group was more obvious,but it was not statistically significant(P>0.05).(3)Comparison of morphological changes of gastric antrum in each group:Visual observation: normal group of rats: gastric mucosa tissue is light red,elastic,wrinkled neatly regular,moderate thickness,smooth mucosa without bleeding,no swelling,no erosion,no ulcers.In the model group,the color of the gastric mucosa was pale,the thinned mucous membrane was not smooth,and even the ulcer was ulcerated.There were cysts,wrinkles and disordered,and even disappeared,and the mucosal fragility increased.Compared with the model group,the rats in the separated cake moxibustion group and the western medicine group had different degrees of improvement.Observation by light microscopy: The intrinsic gland structure of the upper mucosa of the normal group was neatly arranged,the shape was regular,the density was uniform,and the inflammatory cells such as neutrophils and lymphocytes were rare in the mucosa;the intrinsic gland structure of the model group was disordered.Body reduction,thinning,different forms,alveolar structure,cystic dilatation,visible goblet cells,atrophy,intestinal metaplasia and/or dysplasia,and even 2 rats reach poorly differentiated adenocarcinoma,A large number of neutrophils,lymphocytes,and monocytes are seen in the mucosa and submucosa.After 4 weeks of treatment, compared with the model group,the intrinsic glands of the rats in the separated moxibustion group and the western medicine group were irregularly arranged,and the mucosal layer and the lower layer of inflammatory cells were relatively reduced,and the intrinsic gland atrophy,intestinal metaplasia,dysplasia Reduced.(4)The effect of separate cake moxibustion on the expression of PTEN,p-akt,caspase 9,PIP2 and MDM2 proteins in gastric antrum of each group.PTEN: compared with the normal group,the expression of PTEN protein in the gastric antrum of CAG rats was significantly down-regulated,and the difference was statistically significant(PM < 0.001).Compared with the model group,the expression of PTEN protein in the gastric sinus tissues of the western medicine group and the moxibustion group with medicine cake was increased,both of which had statistical significance(P <0.05).P-akt: compared with the normal group,the positive expression of p-akt protein in the gastric sinus tissue of CAG rats increased significantly,and the difference was statistically significant(PM < 0.001).Compared with the model group,the positive expression of p-akt protein was down-regulated in the gastric antrum tissues of the western medicine group and the moxibustion group with medicated cakes,with statistical significance(PWM < 0.001,PHM < 0.001).There was no significant difference in the positive expression of p-akt between the western medicine group and the moxibustion group(PHM=0.639).PIP2: compared with the normal group,the expression of PIP2 protein in the gastric antrum of CAG rats was significantly up-regulated,and the difference was statistically significant(PM < 0.001).Compared with the model group,the expression of PIP2 protein in gastric sinus tissues of the western medicine group and the moxibustion group with medicine cake was significantly decreased,both of which had statistical significance(P <0.05).Caspase-9: compared with the normal group,the expression of caspase-9 protein in gastric sinus tissue of CAG rats was significantly reduced,and the difference was statistically significant(PM < 0.001).Compared with the model group,the expression of caspase-9 protein in the gastric sinus tissues of the western medicine group and the moxibustion group with medicine cake was increased,both of which were statistically significant(P <0.05).MDM2: compared with the normal group,the expression of MDM2 protein in CAG rats increased,and the difference was statistically significant(PM<0.001).Compared with the model group,the expression of MDM2 protein in gastric antrum tissues of the western medicine group and the septum cake group was significantly decreased,and the differences were statistically significant(P<0.001).(5)The effect of separate cake moxibustion on the expression of PTEN,PI3 KCa,caspase-9 and P53 m RNA in gastric antrum of rats in each group.Caspase-9: Compared with the normal group,the expression level of Caspase-9 m RNA in gastric antrum of CAG rats decreased significantly,the difference was statistically significant(P<0.05).Compared with the model group,the western medicine group and the herbal cake-separated moxibustion group rats,the expression of Caspase-9 m RNA in gastric antrum was significantly up-regulated,and the difference was statistically significant(P<0.05).PTEN: Compared with the normal group,the PTEN m RNA of gastric antrum tissue of CAG rats showed a downward trend,but the difference wasn't statistically significant(P=0.058).Compared with the model group,the expression level of PTEN m RNA in gastric antrum tissue of western medicine group and separated medicine moxibustion group The difference between the western medicine group and the western medicine group was statistically significant.There was no significant difference between the medicine and moxibustion group(PWM=0.001,PHM=0.078).PI3KCA: Compared with the normal group,the expression of PI3 KCA m RNA in the gastric antrum of CAG rats increased,and the difference was not statistically significant(PM=0.568).Compared with the model group,the expression of PI3 KCA m RNA in the gastric antrum of the Western medicine group and the herbal cake-separated moxibustion group The level was up-regulated and the difference was statistically significant(PWM=0.016,PHM=0,004).P53: Compared with the normal group,the expression of P53 m RNA in gastric antrum of CAG rats increased,but the difference was not statistically significant(P=0.384).Compared with the model group,the expression of P53 m RNA in the gastric antrum group showed a downward trend.There was no significant decrease in the expression of P53 m RNA in the gastric antrum tissue of the western medicine group(PWM=0.433,PHM=0.769).Conclusion1.Moxibustion with medicated cakes on "zhongwan and qihai" acupoints can improve the general situation of CAG rats,improve the histopathological changes of gastric antrum,repair the damage status of gastric mucosa,and prevent the occurrence of precancerous CAG lesions.2.The "Zhongwan and Qihai" points of the cake-separated moxibustion can reduce the expression of p-akt,PIP2,MDM2 protein and abnormally elevated P53 m RNA in the gastric antrum of CAG model rats,and up-regulate the abnormality of gastric mucosa in CAG rats.Expression of PTEN,Caspase-9 m RNA and protein.3.By regulating the PTEN/PI3K/Akt signaling pathway in the gastric antrum of CAG rats,moxibustion with medicated cakes can effectively control the gastric mucosal injury of CAG,which is one of the effective mechanisms of moxibustion with medicated cakes to improve CAG and prevent and treat precancerous lesions.
