objective: to observe the prevention and treatment effects of Zheng 3 fumigation medicine on articular adhesion after anterior cruciate ligament reconstruction(ACLR)in rabbits,and its effects on the expression of basic fibroblast growth factor(bFGF)and the proliferation of fibroblasts in the adhesion tissues,and to explore the mechanism of zheng 3 fumigation medicine in the prevention and treatment of articular adhesion.Methods: eighteen New Zealand rabbits were randomly divided into three groups: group A(ACLR+ zheng 3 fumigation medicine),group B(ACLR + normal saline),and group C(simple ACLR),with 6 rabbits in each group.After the operation,the knee joint was fixed in A straight position,and on the 4th day after the operation,treatment intervention was carried out in groups A and B respectively,each time for 20 minutes,3 times A week,lasting for 4 weeks.After the intervention,general observation,knee flexion Angle measurement,histological observation,count of fibroblasts and inflammatory cells,and b FGF expression were detected.Results: 1.General observation: after knee joint modeling,there was no obvious abnormal activity,mental state,diet and excretion in rabbits.During the experiment,the plaster fixation of the knee joint on the operative side was not loose and was effective.After knee joint operation,there were no joint infection,bleeding or wound nonunion.2.Knee flexion Angle measurement: the flexion angles of groups A,B and C were 92.33°±6.05°,69.67°±4.84° and 64.33°±4.92°.Under the same traction force(100g),the range of motion of groups B and C was significantly limited.Compared with group B and C,the buckling Angle and activity range of group A increased significantly(P<0.05).There was no significant difference between groups B and C(P>0.05).3.Histological observation with HE staining: in group A,there was A small amount of fibrous hyperplasia,with more fat cells,increased number of ovoid fibroblasts in the nucleus,and A small amount of inflammatory cells infiltrating,mainly lymphocytes.In group B,different degrees of collagen fiber hyperplasia were observed,and different amounts of fibroblast hyperplasia were observed.Multiple inflammatory cell infiltrations,including lymphocytes,neutrophils,and macrophages,were observed.In group C,there was a large amount of fibrous hyperplasia,fibroblast hyperplasia and inflammatory infiltration.4.The count of fibroblasts and inflammatory cells in the 400 x visual field was 10.05±2.28 and 11.16±3.66 in A,32.72±3.45 and 32.61±2.82 in B,35.50±2.77 and 34.55±2.14 in C.Compared with groups B and C,the number of fibroblasts and inflammatory cells in the adhesion tissues of the knee joint of rabbits in group A decreased significantly(P<0.05).Compared with group C,the number of B component fibroblasts and the number of inflammatory cells decreased,but the difference was not significant(P>0.05).5.BFGF expression: the concentrations of bFGF in groups A,B and C were 34.63±1.62pg/mL,38.66±0.97pg/mL and 40.03±1.61pg/mL.Compared with group B and C,the expression of bFGF in the adhesive tissues of the knee joint of rabbits in group A was significantly reduced(P<0.05).There was no statistically significant difference in bFGF concentration between groups B and C(P>0.05).Conclusion:1.Zheng 3 fumigation medicine has certain prevention and treatment effect on joint adhesion after ACLR of knee joint in rabbits,and can delay the occurrence and development of knee joint adhesion to a certain extent.2.The prevention and treatment of adhesion by Zheng 3 fumigation medicine is related to the inhibition of fibroblast proliferation and the reduction of inflammatory infiltration.3.The prevention and treatment mechanism of Zheng 3 fumigation medicine on adhesion may be related to the inhibition of b FGF expression.4.Fixation in rabbits more than 4 weeks after ACLR results in knee adhesion.
