| Purpose:In this experiment,we observed that Longdan Xiegan Tang Decoction combined with Western medicine was used to treat EH patients with hyperactive liver-fire syndrome,to observe the change of blood pressure,heart rate,integral of TCM Syndrome and safety index before and after taking medicine,and to provide effective theoretical basis for clinical treatment of hyperactive liver-fire hypertension.Material and method:The subjects of this study were patients from November 2018 to November 2019 who attended the Specialist Clinic of the Liaoning University of Traditional Chinese Medicine,there were 80 patients who met the diagnostic criteria of "hyperactivity of liver-fire syndrome",diagnosed of stage 2or 3 hypertension in Western medicine.All patients were randomly divided into the experimental group and the Control Group(40 cases in each group).Treatment: Control Group: Benidipine hydrochloride tablets 2 tablets per day(2mg),plus sotalol tablets 1 tablet per day(80mg).Test Group: On the basis of the control group,add and subtract decoction of Longdan Xiegan Tang Decoction:Gentian(stir-fried with wine)15g,Huangqin(stir-fried with wine)15g,Gardenia 10 g,Prunella Vulgaris 10 g,Plantain Seed 10 g,Cassia Seed 10 g,Angelica(stir-fried with wine)6g,Radix Rehmanniae(stir-fried with wine)10g,Radix Bupleuri 10 g,Prepared Licorice 6g.Take orally,one dose daily,three times a day(100 ML each).The course of treatment was 4 weeks in both groups.To evaluate the clinical efficacy of the two groups: Before Treatment(0 Weeks,representing the time before treatment)and after treatment(4 weeks,representing the time at the end of treatment)twenty-four hours of mean blood pressure,day and night mean blood pressure,twenty-four hours of blood pressure variability,daytime and nighttime blood pressure variability,twenty-four hours of mean heart rate,day and night mean heart rate,the two groups of patients before and after treatment of TCM Syndrome integral efficiency,Chinese medicine single syndrome integral situation and so on.Results:1.TCM symptom score:The total effective rate of TCM syndrome integral was91.67% in the experimental group and 80.56% in the control group,there was a significantdifference(P=0.009).The improvement of the total score of TCM syndrome: The total score of TCM Syndrome before treatment was 20.28±3.36 and 7.44 ±3.29 after treatment in the experimental group,The total score of TCM Syndrome before treatment was20.03 ±2.90 and10.08 ±4.77 after treatment in the control group,there was a significant difference(P=0.008).The scores of TCM Primary Single Syndrome(Headache,Dizziness,Irritability)in the two groups were improved after treatment(P< 0.05),and the improvement of Headache in the test group was better than that in the control group(P=0.035),the improvement of the scores of the other symptoms in the two groups was similar(P> 0.05).;After treatment,all the secondary symptoms of the test group were improved obviously(P<0.01),and all the secondary symptoms of the control group were improved(P<0.05)except the Facial redness(P=0.064),the improvement of the Urine fever in the test group was better than that in the control group(P=0.041),the improvement of the scores of the other symptoms in the two groups was similar(P> 0.05).2.24 h ambulatory blood pressure:Mean systolic blood pressure at 24 h,mean diastolic blood pressure at 24 h,mean systolic blood pressure at night,mean diastolic blood pressure at night,mean systolic blood pressure at day and mean diastolic blood pressure at day in both groups were lower than those before treatment,there were significant differences(P<0.05).After treatment,there were significant differences in 24 h mean systolic pressure,24 h mean diastolic pressure,night mean systolic pressure and night mean diastolic pressure between the two groups(P< 0.05).However,there was no significant difference between the two groups in daytime mean systolic pressure and daytime mean diastolic pressure(P> 0.05).3.The Coefficient of blood pressure variation(CV): After treatment,24 h SBP-cv,24 h DBP-cv,d SBP-cv,d DBP-cv,n SBP-cv and n DBP-cv were significantly lower than those before treatment(P=0.000).After treatment,there were significant differences between the two groups in 24 h SBP-cv,24 h DBP-cv,n SBP-cv and n DBP-cv(P<0.05).There was no significant difference between the two groups in d SBP-cv(P=0.457),d DBP-cv(P=0.319).4.Heart rate:The mean heart rate at twenty-four hours,the day average heart rate and the night average heart rate after treatment were significantly lower than those before treatment(P =0.000).After treatment,the mean heart rate at twenty-four hours and the mean heart rate at night were significantly different between the two groups,the differences were statisticallysignificant(P < 0.05),and the mean heart rate during the day was compared between the two groups,there was no statistical difference(P=0.316).Conclusion:1.Longdan Xiegan Tang Decoction combined with conventional basic treatment of Western medicine can effectively reduce blood pressure in patients with hyperactive liver-fire Hypertension,the curative effect is better than simple conventional basic treatment of Western medicine.2.Longdan Xiegan Tang Decoction combined with conventional basic treatment of Western medicine could relieve headache,dizziness,irritability and urine fever in patients with hyperactive liver-fire Hypertension.3.Longdan Xiegan Tang Decoction combined with Western medicine can further reduce the blood pressure and the coefficient of blood pressure variation in the whole day and at night,and thus reduce the fluctuation of blood pressure.4.Longdan Xiegan Tang Decoction combined with conventional basic treatment of Western medicine can further reduce the average heart rate of twenty-four h and the average heart rate at night.5.Longdan Xiegan Tang Decoction without adverse reactions and side effects,safe and effective,worthy of clinical application. |
