Research On The Distribution Of TCM Syndrome Types Of Arrhythmia And The Law Of Prescriptions And Medicines

Objective: Data mining method is used to analyze and summarize the common syndrome types,syndrome elements and distribution rules of the application of prescriptions in Chinese medicine for arrhythmia,so as to provide guidance for clinical diagnosis and treatment.Methods:1.Select the clinical research literature of arrhythmia in China Zhiwang,VIP database and Wanfang Data knowledge service platform from 1989 to 2019,extract the TCM syndrome type,disease location,pathogenic syndrome elements and prescriptions of arrhythmia,and analyze the distribution of TCM syndrome type,syndrome elements and prescriptions of arrhythmia and the combination rule of syndrome elements by using IBM SPSS statistics21.2.By retrospective analysis,179 patients with a rrhythmia who were hospitalized in the cardiovascular department of the First Affiliated Hospital of Hunan University of traditional Chinese medicine from January 2015 to October 2019 were included.All the case data were directly entered into "Chinese Medicine Inheritance auxiliary platform v2.5".Using the data mining function of the platform,through frequency statistics,correlation analysis and entropy based method,the rules of prescription composition were analyzed Analysis and so on,summarizes the arrhythmia Chinese medicine symptom,the syndrome type distribution and the prescription medicine utilization distribution rule and the characteristic.Results:1.138 papers about arrhythmic syndrome types were screened out,including 25 syndrome types,6 disease location syndrome elements,11 disease location combination syndrome elements,10 disease type syndrome elements,15 disease type combination syndrome elements and 33 prescriptions.The common syndrome types are: deficiency of heart qi and Yin,de ficiency of heart qi and blood,disturbance of heart by phlegm and fire,deficiency of heart Yang,deficiency of heart kidney yang,etc.the syndrome elements of disease location are mainly heart,spleen,liver,kidney,the syndrome elements of disease location combination are mainly heart spleen,heart kidney,heart liver,spleen kidney,etc.the syndrome elements of disease location combination are mainly Yang deficiency,Qi deficiency,phlegm dampness,blood stasis,qi stagnation,blood deficiency,etc The main prescriptions are Zhigancao Decoction,Guipi Decoction,Xuefu Zhuyu Decoction,Shengmai Powder,Huanglian Wendan Decoction,Gualou xiebanxia decoction,etc.2.The clinical research shows that: the ratio of male and female patients with arrhythmia is 1:1.71,which indicates that the number of female patients with arrhythmia is higher than that of male patients;the age distribution shows that the minimum age of the patients is 19 years old,and the maximum age is 85 years old.According to the comprehensive analysis,the incidence of arrhythmia mainly comes from the middle-aged and elderly women over 40 years old.Symptom distribution: palpitation,chest distress,insomnia,dizziness,fatigue,dry mouth,poor appetite,cough,shortness of bre ath,sweating,etc.are more common;tongue pulse is more common in red tongue,light red tongue,yellow and greasy fur,thin and white fur,stringy pulse,etc.According to the frequency of syndrome differentiation and classification,it is shown that: phlegm and fire disturb the heart syndrome > deficiency of heart blood syndrome > deficiency of Qi and Yin Syndrome > stagnation of heart pulse syndrome > stagnation of heart Yang Syndrome > deficiency of yin and fire hyperactivity syndrome > stagnation of l iver Qi syndrome;there are 203 traditional Chinese medicines used in the treatment of arrhythmia,and through the statistical analysis of the auxiliary platform of traditional Chinese medicine inheritance,there are 30 commonly used drugs,mainly Poria co cos,FA Pinellia,angelica,Chenpi Calcined oyster,roasted licorice,Suanzaoren,vinegar bupleurum,calcined keel,Danshen,astragalus,white peony,chuanxiong,etc.can be divided into expectorant,stasis,Qi,calming,Qi,Yin.Through data mining,28 kinds of core drug combinations were obtained according to association rules,including forged keel and oyster,FA Pinellia and tangerine peel,Chen peel and poria,FA Pinellia and poria,Suanzaoren and Yuanzhi,etc.;according to clustering algorithm analys is,drug combinations were angelica and Atractylodes macrocephala,chuanxiong and Fructus aurantii,Mai Dong and Fructus aurantii,Mai Dong and tangerine peel,Mai Dong and longan meat,etc.According to the entropy clustering of complex system,28 kinds o f combination forms were obtained,such as Zhuru + Zhishi + Fuling,forge keel + Scutellaria + forge oyster,stir fried malt + Shenqu + Yunzhi,Maidong + Tiandong + Schisandra,Shenqu + Atractylodes Rhizoma + turtle shell,prepared rehmannia + peony skin + jinneijin,Tiandong + Xuanshen + Platycodon,etc.;according to the entropy level clustering,14 new prescriptions for the treatment of arrhythmia were obtained,such as FA Banxia + Huangqin + Angelica + Zhishi + Huanglian,Mai Banxia Winter + asparagus + Schisandra + Fructus Trichosanthis + Radix Pseudostellariae,Radix Paeoniae Rubra + vinegar Xiangfu + peach kernel + frankincense + Ligusticum chuanxiong + Achyranthes bidentata + red flower,prepared rehmannia + peony skin + chicken internal gold + yam + Cornus,calcined keel + Scutellaria + calcined oyster + aconite,etc.Conclusion:1.Through literature search,the distribution of arrhythmia syndrome types,disease location,characteristics of disease syndrome elements and the application of prescriptions were clarified,providing reference for clinical treatment of arrhythmia.2.Arrhythmia occurs frequently in the middle-aged and elderly women over 40 years old.The main symptoms are palpitation,chest distress,insomnia,dizziness and fatigue.The syndrome differentiation types are phlegm and fire disturbing heart syndrome,heart blood deficiency syndrome and Qi Yin deficiency syndrome.The pathogenesis changes are closely related in medication.Through the data mining of clinical cases,we can get th e characteristics of TCM symptoms,syndrome type distribution,core combination and new prescription of commonly used drugs,optimize TCM syndrome differentiation and treatment thinking,standardize the diagnosis and treatment process of arrhythmia,and gi ve full play to the flexibility and diversity of TCM treatment.
