The Curative Effect Observation Of Arthroscopic Rotator Cuff Repair Combined With Acupuncture In The Treatment Of Rotator Cuff Tear

Purpose: To observe the clinical effect of combined acupuncture and moxibustion for rotator cuff tear after shoulder arthroscopy.Material and method:From June 2016 to February 2018,40 patients with rotator cuff tear were admitted to Department of Orthopaedics,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine.They were diagnosed as rotator cuff tear under arthroscope and were treated with rotator cuff repair(double-row suture bridge fixation)under arthroscope.There were 16 male patients and 24 female patients,aged 27-65 years,with an average age of 54.55 years.According to the De Orio and Caofield grades commonly used clinically,the injury types were divided into mild laceration(< 1 cm),moderate laceration(1-3 cm),large laceration(3-5 cm)and giant laceration(>5 cm),of which 5 cases were mild laceration,29 cases were moderate laceration,6 cases were large laceration and 0 cases were giant laceration.Has undergone shoulder surgery.All patients were randomly divided into the control group and the experimental group,20 cases in each group.The control group was treated only by arthroscopic shoulder surgery(double-row suture bridge fixation).After the operation,symptomatic treatment such as anti-inflammatory,thrombosis prevention,analgesia,braking and rehabilitation exercise were routinely applied.The experimental group was treated with acupuncture and moxibustion in three cycles on the basis of the same treatment plan of the control group after the operation.The first cycle was 4 weeks after the operation.Shoulder condyle,shoulder condyle,shoulder Zhen3 points with Houxi point,Shugu point for acupuncture,the second cycle: 3 months after the operation choose shoulder condyle,shoulder condyle,shoulder Zhen3 points with Tianzong point,Yanglingquan point for acupuncture,the third cycle: 6 months after the operation choose shoulder condyle,shoulder condyle,shoulder Zhen3 points with Tiaokou point,Zusanli point for acupuncture.The patients were treated with needle retention for 20 minutes,once a day,15 times per cycle.The pain and shoulder function were evaluated by Visual Analogue Pain Scale(VAS)and UCLA before,6 weeks,6 months and 12 months after the operation,and the statisticalanalysis was made.Results:1.The follow-up rate of 40 patients was 100%,and the follow-up time was 6 weeks after operation,6 months after operation and 12 months after operation.2.The VAS score of the control group before operation(5.00±1.45),6 weeks after operation(4.60±1.14),6 months after operation(3.55±0.89),and 12 months after operation(2.50±0.89).The UCLA score was preoperative(16.80±3.47),6 weeks after operation(15.55±0.99),6months after operation(23.60±1.50),and 12 months after operation(31.15±1.23).3.The VAS score of the experimental group was(4.95±1.47),6 weeks after operation(3.55±1.05),6 months after operation(2.85±0.88),and 12 months after operation(1.95±0.89).The UCLA score was preoperative(17.05±2.61),6 weeks after operation(16.50±1.00),6months after operation(25.15±1.73),and 12 months after operation(32.20±1.51).Conclusion:The effect of acupuncture combined with acupuncture on rotator cuff tear is better than that of shoulder arthroscopy alone.Combined acupuncture treatment after shoulder arthroscopy not only has excellent analgesic effect on patients after operation,but also has obvious promotion effect on functional recovery after rehabilitation.
