Objective To observe the effect of auricular acupoint pressing beans on rehabilitation after rotator cuff repair under shoulder arthroscopy,so as to provide a new choice for rehabilitation after rotator cuff repair. Methods From June 2017 to September 2018,58 patients who met the inclusion criteria for rotator cuff repair under arthroscopy in the Bone Trauma Center of the First Affiliated Hospital of ** were divided into experimental group(29cases)and control group(29 cases)according to the digital table.Finally,27 cases in experimental group and 22 cases in control group were collected. The experimental group was treated with auricular point pressing method combined with routine rehabilitation after operation, while the control group only used routine rehabilitation method after operation.Visual analogue scale was used to record and compare the pain of two groups at different time points after operation;the pressing times of intravenous self-controlled analgesia pump in different time periods of 0-12 h,12-24 h and24-48 h were recorded and compared between the two groups;the occurrence of nausea, vomiting, sleepiness, skin itching and dizziness and headache within 48 hours after operation were recorded and compared between the two groups. The shoulder Constant-Murely score was recorded before,3,6 and 12 weeks after operation. Results 1.Comparison of VAS scores at different time points after operation in the two groups: There was no interaction between the two groups(F = 0.834,P = 0.366)by repeated measurement variance analysis;VAS scores at different time points after operation in the two groups had time effect(F = 154.216,P = 0.000);VAS scores at different time points after operation in the two groups had group effect(F = 18.443,P = 0.000).There was no significant difference in pain scores between the experimental group at 12 h,24h,48 h,6w and 12 w after operation(P > 0.05).The VAS score between 3 d and 3w after operation showed significant difference between the two groups(P < 0.05).2.Comparison of pressing times of analgesic pump in two days after operation between the two groups: Comparisons of pressing times of analgesic pump in 0-12 h,12-24 h and 24-48 h between the experimental group and the control group: The average number of pressing timesof analgesic pump in the experimental group was less than that of the control group in three periods, and there was statistical difference(P < 0.05).3.Comparison of adverse reactions to analgesia within two days after operation between the two groups: The incidence of adverse reactions after operation in the experimental group was7.4%,including nausea in 1 case, dizziness and headache in 1 case. The incidence of adverse reactions in the control group was 36.4%,including nausea in 4 cases, vomiting in 1 case, dizziness and headache in 3 cases. The adverse reactions in the experimental group were significantly less than those in the control group, and the incidence of adverse reactions was significantly different(P < 0.05).4.Comparisons of abduction and rotation scores before and after operation at 3,6 and 12weeks: There was no interaction between the two groups(F = 0.477,P = 0.700)and abduction-rotation scores in two groups in the two groups had group effect(F = 10.071,P = 0.003).Constant-Murley's abduction-rotation score had a time effect(F = 163.069,P = 0.000).The scores of both groups showed an upward trend with time, but the upward trend was different. There was no significant difference between the two groups before and 12 weeks after operation(P > 0.05),but there was significant difference between the two groups at 3 and 6weeks after operation(P < 0.05).5.ADL item scores of the two groups before and 3,6 and 12 weeks after operation: Statistical analysis of repeated measurements of variance showed that there was no interaction between the two groups(F = 1.798,P = 0.161);there was grouping effect in ADL item scores of the two groups(F = 5.749,P = 0.021).There was a time effect on ADL item score in both groups(F = 179.453,P = 0.000);ADL score in both groups showed an upward trend, but the upward trend was inconsistent. There was no significant difference in ADL score between the two groups before operation,3 weeks and 12 weeks after operation(P > 0.05),but there was significant difference between the two groups at 6 weeks after operation(P < 0.05).6.The total Constant-Murley scores of the two groups before and after operation for 3 weeks,6 weeks and 12 weeks: Statistical analysis of repeated measurements of variance showed that there was an interaction between the two groups(F = 3.678,P = 0.016);there was a grouping effect in the Constant-Murley scores of the two groups before and after operation for 3 weeks,6 weeks and 12 weeks(F = 11.466,P = 0.001);and there was a time effect at before and after operation for 3 weeks,6 weeks and 12 weeks(F = 922.467,P = 0.000).The total Constant-Murley score of the two groups showed an upward trend, but the upward trend was different;there was no difference between the two groups before operation and 12 weeks after operation(P > 0.05);there was a significant difference between the two groups at 3 and 6 weeks after operation(P < 0.05).Conclusion Auricular point pressing beans method can not only effectively relieve the early pain after shoulder cuff repair under arthroscope, reduce the use of analgesics, reduce the adverse reactions caused by analgesics, and is beneficial to the recovery of patients after operation, but also play a role in relieving pain in the process of functional rehabilitation after operation, and promote patients to actively carry out functional rehabilitation exercises, thus improving the work of shoulder joint of patients. It can improve the quality of life of patients, improve the satisfaction of patients, and has no other obvious adverse reactions. It is a safe and effective treatment method.
