Due to the marketization of medical and health reform,the distribution of physician resources in different regions(urban and rural),different grades and different nature(public and private)is unbalanced,and the overall allocation efficiency of human resources is not high.Meanwhile,because of the medical and health personnel system,the mobility of doctors between different medical institutions is limited.The difficulty of seeing a doctor has not been effectively alleviated owing to the inadequate support of high-level doctors to primary medical institutions.Therefore,improving the quality and efficiency of medical services is one of the people's expectations for the new health care reform,and allowing doctors to practise more is one of the policies to respond to people's expectations in the context of the continuous advancement of the new health care reform.Chengdu,as one of the first pilot cities for multi-point practicing of physicians,issued the "Chengdu multi-point practicing pilot work program" and " Chengdu Multi-point Practicing Pilot Work Management Measures " in August 2012.Subsequently,Qingbaijiang District began to promote this work by issuing papers one after another.In terms of the advantages of the policy itself,the multi-point practicing policy of physicians helps to balance medical resources and promote the exchange and smooth and orderly flow of medical resources among different medical institutions.At the same time,it can improve the management mechanism of public medical institutions and improve the medical level and quality of medical services of primary medical and health units.However,according to the multi-point registration of doctors in Qingbaijiang District,the effect of this policy in Qingbaijiang District is unsatisfactory.Mainly reflected in: the compliance of policy target group is poor,the proportion of doctors participating in multi-point practice is far from enough,and there is a certain gap between the choice of doctors' multi-point practice location and policy expectations.In the meantime,the implementation of the multi-point practicing policy for doctors in Qingbaijiang District is also facing many difficulties,which hinders the expected role of the multi-point practicing policy for doctors.This paper synthetically uses the research methods of literature research,questionnaire survey and in-depth interview,from the perspective of institutional environment,supporting policies,and interest correlation.To find out the problems existing in the multi-point policy of doctors in Qingbaijiang District,such as the waiting-and-seeing state of doctors,the inconsistency between multi-point practice places and target expectations,the selective execution of medical institutions and the lack of supervision by health administrative departments,and to analyze the causes of the problems Using Smith Analysis Model.By referring to the advanced work experience at home and abroad and different medical practicing modes in China,and combining with the actual situation of medical and health work in Qingbaijiang District,this paper puts forward some countermeasures and suggestions for the smooth implementation of the multi-point practicing policy for doctors in Qingbaijiang District which includes:Promoting the reform of health personnel system,perfecting the supporting system of multi-point practicing,and promoting the implementation of classification of multi-point practicing of physicians,strengthening the management and control of multi-point licensed physicians,speeding up the construction of grass-roots health service system,encouraging hospitals to innovate system management and promoting standardized training of physicians.
