Background:Tendonitis of long head of biceps brachii is one of the most common causes of anterior shoulder pain in clinical work,which often occurs in the form of secondary changes,due to the similar clinical manifestations and the lack of sensitivity and specificity in physical examination,it is often confused with other shoulder diseases.and patients often pay insufficient attention to this disease.which will seriously affect their life and work over time.In the past,the application of non-steroidal anti-inflammatory drugs and corticosteroids has obvious side effects in the long term,and surgical treatment has problems of postoperative complications,which also puts forward certain requirements for the risk tolerance of patients.It has become particularly necessary to find a method that combines accurate diagnosis with effective treatment.Objective:To observe and evaluate the improvement of shoulder pain and activity function in patients with biceps brachii tendinitis by using ultrasound positioning needle knife,systematically evaluate the clinical efficacy of' ultrasonic localization needle knife in the treatment of biceps tendonitis.To summarize the causes and characteristics of pathological changes under ultrasound,measure the angle of the inner wall of the nodular sulcus,provide imaging support for the diagnosis of the disease,and promote the treatment of biceps femoritis with ultrasound positioning needle knife.Methods:The patients with biceps tendonitis in the orthopedic clinic of the Third Affiliated Hospital of Beijing University of Chinese Medicine from January 20 to 2019 were treated with ultrasound positioning needle knife treatment for 34 patients with biceps tendonitis.The patients' VAS score and Constant-Murley score were recorded before operation.I week after operation,2 weeks after operation and 1 month after operation.The statistical data were processed by Spss20.0 software package,and the curative effect was evaluated.At the same time,the performance under ultrasound was observed and recorded,and the angle between the inner walls of the nodule was measured.Results:1.Of the 34 patients.15 were male accounting for 44.12%,and 19 were female,accounting for 55.88%.All patients had pathological changes in 29 patients with fluid dark area(effusion),accounting for 85.29%.liquid depth 0.24-1.33cm;7 patients with tendon sheath thickening,accounting for 20.59%;1 patient with tendon thickening,accounting for 2.94%;3 patients with localized calcification,accounting for 8.82%.2.Four sets of VAS scores and Constant-Murley score data corresponding to the four time nodes were compared and analyzed in a comprehensive way:two patients were treated with VAS scores before treatment.after I week of treatment,after 2 weeks of treatment and after 1 month of treatment.Two paired sample rank sum tests,preoperative>1 week after surgery>2 weeks after surgery>1 month after surgery,P<0.05,the difference was statistically significant,indicating that after 1 week.2 weeks,and 1 month after treatment Ultrasound positioning needle knife still has an effect on the improvement of pain;the variance analysis of the Constant-Murley score and the one-way repeated measures before and after treatment,after 1 week of treatment,after 2 weeks of treatment and after 1 month of treatment,showed There was a statistically significant difference in the Constant-Murley score between the first week and the second week after surgery,one week after surgery,one month after surgery,and two weeks after surgery(P<0.05).By comparing the mean values.we believe that the shoulder joint function is gradually improved.3.Among the 34 patients,4 were cured,accounting for 11.11%,26 were markedly effective,accounting for 80.560%,2 were effective,accounting for 5.56%,and 2 were ineffective,accounting for 5.56%.The total effective rate was 94.12%.4.The angle of the inner wall of the nodular sulcus(? angle)was measured in 34 patients,including 1 type ?,15 type ?,10 type ?,7 type ?,? type ?,and 0 type ?.Conclusion:1.Ultrasound positioning needle knife is used to treat shoulder pain and improve shoulder movement function in patients with biceps femoritis.2.The pathological changes of simple biceps femoris tendinitis mainly include effusion.tendon thickening,tendon sheath thickening and calcification,mainly due to effusion changes.3.Among the 34 patients,the angle of the inner wall of the internodal sulcus was mainly type ? and type ?,and it was easy to develop biceps lendonitis under 45 degrees.
