Objective:To observe and analyze the clinical effect of ultrasound-guided acupotomy combined with shock wave in the treatment of long head tendinitis of biceps brachii.To explore the feasibility of ultrasound-guided treatment and efficacy evaluation of long head biceps tendinitis.It was verified that ultrasound-guided acupotomy combined with shock wave had better clinical effect in the treatment of long head biceps tendinitis.Methods:From September 2021 to December 2022,60 patients diagnosed with tendinitis of long head of biceps brachii tendons in the orthopedic Clinic of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine were collected and divided into the treatment group and the control group by random number table method,with 30 patients in each group.The treatment group was treated with ultrasound-guided acupotomy combined with shock wave therapy,while the control group was treated with acupotomy combined with shock wave therapy based on experience and anatomical positioning.The Visual Analogue Scale(VAS),Constant-Murley Score(CMS)and ultrasonic measurements of biceps long head tendon,tendon sheath thickness and maximum diameter of tendon sheath effusion were analyzed and compared between the two groups before treatment and 1 and 4 weeks after treatment.And the clinical effect after 4 weeks of treatment.Results:1.VAS score:VAS scores of 2 groups were decreased to varying degrees after1 week and 4 weeks of treatment,and symptoms were relieved,the difference between 2 groups was statistically significant(P< 0.05),indicating that the pain relief effect of the treatment group was better than that of the control group;2.CMS score: CMS scores in both groups increased after 1 week and 4 weeks of treatment,and shoulder joint function of patients improved to different degrees compared with before.Comparison between the two groups showed that the treatment group was superior to the control group,and the difference was statistically significant(P< 0.05);3.Ultrasonic measurement:After 1 week and 4 weeks of treatment,compared with before treatment,the thickness of biceps long head tendon,tendinous sheath and the maximum diameter of tendinous sheath fluid of the two groups were decreased,and the results of the treatment group were better than the control group,the difference was statistically significant(P< 0.05);4.Clinical effective rate analysis after 4 weeks of treatment showed that the total effective rate of the treatment group was 93.33% and that of the control group was 90%,and the difference was statistically significant(P < 0.05).Both groups were effective,but the effective rate of the treatment group was higher.Conclusion:1.Ultrasound-guided acupotomy combined with shock wave therapy is superior to traditional an anatomical orientation combined with acupotomy combined with shock wave therapy in the treatment of long head biceps brachii tendinitis,indicating that ultrasound-guided acupotomy combined with shock wave therapy can significantly relieve pain,improve shoulder joint function and improve therapeutic effect.2.Ultrasonic-guidance enables the visualization of acupotomy operation,which can accurately locate the lesion,provide accurate approach for acupotomy operation,avoid unnecessary damage to the surrounding soft tissue,and achieve accurate treatment.
