Research backgroundChronic subjective dizziness(CSD)is a chronic non-rotation property subjective dizziness disease,which seriously affects the quality of life and daily life of patients.More and more data show that anxiety,depression and other emotional disorders as well as sleep disorders are one of the important reasons affecting the onset of the disease.In the aspect of traditional Chinese medicine,the researches on thecharacteristics of Vertigo disease's symptomatology and syndrome are mostly focused on"cervical spondylosis","cerebral ischemia","inner ear pathology" and " posterior circulation ischemia".However,there are few reports on the characteristics of TCM syndromes of chronic subjective dizziness with high clinical incidence.Research objectivesThrough data collection and statistical analysis of clinical data,TCM four diagnostic information,mood state and sleep quality of patients with chronic subjective dizziness,the main TCM syndrome characteristics of chronic subjective dizziness are discussed,the relationship between chronic subjective dizziness and mood state and sleep quality is analyzed,and the distribution law related to TCM syndrome is analyzed,which provides clinical basis for timely and accurate identification and diagnosis of chronic subjective dizziness and correct treatment based on syndrome differentiation.Research methodsIn this study,anxiety,depression and sleep quality of patients who meet the diagnosis of chronic subjective dizziness were investigated through the Vertigo Disability Assessment Scale(DHI),14 Hamilton Anxiety Scale(HAMA),17 Hamilton Depression Scale(HAMD)and Pittsburgh Sleep Index Scale(PSQI).According to the description of vertigo symptoms in"internal medicine of Traditional Chinese Medicine" and "Guiding Principles for Clinical Research of New Chinese Medicines" and combined with clinical practice and previous literature research,a "TCM Symptom Element Table for Chronic Subjective Dizziness"was designed to collect TCM four diagnostic information for selected patients.Research results160 cases were included in this study,of which 63 were males,accounting for 39.4%of the total.97 women,accounting for 60.6%of the total.The average age of the patients was(56.35±16.32).Among them,45 patients were younger than 45 years old,accounting for 28.1%.63 patients aged 45-65,accounting for 39.4%;52 patients were over 65 years old,accounting for 32.5%.Among them,there is a big difference between men and women in middle-aged and elderly CSD patients,and women patients are much higher than men.HAMA and HAMD scores of CSD patients of different genders have statistical significance,which indicates that gender is one of the influencing factors of anxiety and depression of CSD patients,and anxiety and depression degree of female CSD patients is higher than that of male CSD patients.There was no significant difference in DHI,HAMA,HAMD and PSQI scores among CSD patients of different ages,which indicated that age was not an influential factor of dizziness,anxiety,depression and sleep quality in CSD patients.The average score of Vertigo Disability Scale(DHI)was 33.72±13.02.The severity of dizziness was mainly mild disorder.The average Hamilton Anxiety Scale(HAMA)score was 8.24 ±3.9,and 92 patients(57.5%)had anxiety.The average score of Hamilton Depression Scale(HAMD)was 6.29±3.80,and 64 patients(40%)had depression.The average score of pittsburgh sleep quality index Scale(PSQI)is 9.86±3.87,and 110 patients(68.75%)have sleep disorders,which indicates that most patients have sleep disorders.Through cluster analysis,158 cases were screened,and chronic subjective dizziness was divided into 5 ty pes of syndromes:liver depression and spleen deficiency type(29 cases,accounting for 18.3%),liver yang hyperactivity type(39 cases,accounting for 24.7%),kidney essence deficiency type(38 cases,accounting for 24.1%),phlegm-dampness type(21 cases,accounting for 13.3%)and deficiency of both heart and spleen type(31 cases,accounting for19.6%).The correlation and difference between different TCM syndromes and dizziness,anxiety and depression,sleep quality have no statistical significance.Research conclusion1.The incidence rate of female patients with chronic subjective dizziness is higher than that of male patients;The incidence rate of elderly and middle-aged patients is higher,followed by young patients.2.The severity of anxiety and depression in female CSD patients is higher than that in male CSD patients.Age is not an influential factor of dizziness,anxiety,depression and sleep quality in CSD patients.3.Anxiety,depression and sleep quality of patients with chronic subjective dizziness are positively correlated with the severity of dizziness.The more serious anxiety,depression and sleep disorder are,the more serious the dizziness is.4.The TCM syndromes of chronic subjective dizziness are classified into:liver yang hyperactivity type,kidney essence deficiency type,liver depression and spleen deficiency type,deficiency of both heart and spleen type,phlegm-dampness type.5.The degree of dizziness,anxiety,depression and sleep disorder of patients with chronic subjective dizziness of different TCM syndromes have no obvious difference.
