A Retrospective Study Of Depression Cases In St. Petersburg Hospital Of Traditional Chinese Medicine And A Meta-analysis Of Clinical Literature On The Treatment Of Depression With Acupuncture And Medicine

Objective To conduct a comprehensive,systematic and standardized medical record retrospective study on the case data of patients with depression in the St.Petersburg Center for Traditional Chinese Medicine from 2015 to 2018.At the same time,a systematic randomized controlled trial of acupuncture combined with conventional medicine for the treatment of depression was systematically evaluated.Methods The medical records of the patients with depression collected in the past three years were summarized and analyzed by retrospective analysis of medical records.Meta-analysis was used to evaluate the current randomized controlled trials of acupuncture combined with depression in the treatment of depression and to make a general analysis of clinical efficacy.Results The gender ratio of the included cases was 1:1.875.showing the characteristics of women over men.The age of the patients was mostly between 41 and 60 years old,accounting for 51.9%of the total.Patients enrolled in the study were prone to disease in summer and autumn(78.2%).The previous medical history of the patients in the analysis was mainly concentrated in three categories of hypertension(39.1%),cholecystitis(30.4%)and thyroid nodules(26.1%).In this study,the physique classification of patients was mainly qi stagnation(30.4%),yin deficiency(26.1%),and phlegm-dampness(26.1%).The liver stagnation accounted for 78.3%of the syndrome type.The liver qi stagnation syndrome(30.4%),liver stagnation syndrome(26.1%),heart and spleen deficiency syndrome(21.7%).In the diagnosis of tongue and pulse,the number of patients with characteristics such as "red","complex" and "stinging" is more,accounting for 91.3%of the total number of patients.The tongue coating is characterized by "greasy" and "less".The above characteristic tongue images have common disease significance.In terms of pulse diagnosis,it can be seen from the collected data that the patient's pulse has the following characteristics:the left-inch pulse is dominated by strings and floats(34.8%),and the left-off pulse is dominated by strings and slides(43.5%).The ruler is mainly Shen and Xuanmai(56.5%);the right vein is the Guanxian(56.5%),the ruled sink(26.0%),the string(17.3%),and the weak(17.3%).The pulse shows the characteristics of the left and right veins,and the left and right veins.In this study,the patient's depression often mixed with other psychiatric symptoms,the first manifestation was sleep disorders(34.7%),followed by fear(17.4%),tension(13.0%),irritability(13.0%)and other emotional disorders.60.9%of patients were associated with facial facial features,mainly head discomfort and ear hearing abnormalities.Dizziness and headache accounted for 39.13%.and tinnitus and hearing loss accounted for 21.7%.Other physical symptoms often associated with patients in this study:fatigue(30.4%)was the lmost common physical condition.followed by muscle soreness(43.5%),tension and stiffness(26.0%)in all parts of the body.A total of 21 patients(91.3%)were accompanied by other visceral symptoms,with chest and abdomen symptoms:chest tightness,heart palpitations accounted for 39.1%,nausea.acid reflux,and abdominal distension were 39.1%.All patients were treated with conventional acupuncture and Chinese patent medicine.In the course of treatment,they used infrared light and moxibustion.Another 7 patients were treated with cupping and fire needles.In the aspect of acupoint selection,the conventional acupuncture treatment mainly selects acupoints with the function of relieving depression and regulating the spirit,and more uses abdominal acupoints and specific points of the extremities.The total utilization rate of five meridians and acupoints such as the foot-yangming stomach meridian.the foot Shaoyang gallbladder.Renmai,the foot Shaoyin kidney,and the foot Taiyin spleen was 72.9%.The highest rate of use of the foot-yangming stomach acupoints was 17.7%,followed by 13.0%of the Shaoyangyang and 14.4%of the veins.The acupuncture points of the auricular acupuncture are relatively fixed.The heart,liver,spleen and kidney are used to adjust the viscera,and the Shenmen,Sympathetic,Endocrine,Thoracic,Cervical,and Lumbar Vertebrae are used according to the patient's condition.Cupping is mainly used to alleviate the discomfort of the neck and shoulder muscles of patients,and the choice of acupoints is mostly selected.Moxibustion uses acupuncture points on the abdomen,and uses the points of the foot Yangming stomach,the foot Shaoyin kidney and the Renmai,Tianshu.Yushu,Shenque,Qihai,Guanyuan,Zhongyu and other acupoints.In terms of efficacy,the patients included in the study had improved depression after receiving treatment at the center.The efficacy was different from baseline after the treatment,and the total effective rate was 86.95%after 5 weeks of treatment.In Study 2,21 studies were eventually included and a total of 451 subjects were included.The experimental group used acupuncture combined with traditional Chinese medicine as an intervention measure,and the control group was treated with conventional western medicine.The results of Meta analysis showed that the efficacy of the acupuncture combined with the experimental group was better than that of the conventional western medicine control group.The total effective rate meta-analysis was[OR=2.95.,95%CI(2.07,4.22)],Z=5.95,P<0.00001;The meta-analysis results were[OR=-3.11,95%CI(-3.51,-2.71)],Z=15.22,P<0.00001;the meta-analysis of the adverse reaction rate was[OR=0.12.,95%CI(0.03,0.42).)],Z=3.31,P=0.0009.the above differences were statistically significant.Conclusion In general,the depression patients included in the study 1 showed that women were more than men,and the incidence of the disease was mainly in summer and autumn.The patient's mostly chest and abdomen.In terms of treatment.all patients used conventional acupuncture and Chinese patent medicine.In the aspect of acupoint selection,five meridians including foot Yangming stomach,foot Shaoyang gallbladder.Renmai,foot Shaoyin kidney,and foot Taiyin spleen were used.Acupoints.Infrared light and moxibustion are used in the treatment process.Another 7 patients were treated with cupping and fire needles.The acupuncture points are selected from the four points of heart,liver,spleen and kidney,and according to the patient's condition,Shenmen,sympathetic,endocrine,thoracic vertebrae,cervical vertebrae,lumbosacral vertebrae and other points are used.Cupping is mainly used to alleviate the discomfort of the neck and shoulder muscles of patients,and the choice of acupoints is mostly selected.Moxibustion uses acupuncture points on the abdomen.and uses the points of the foot Yangming stomach,the foot Shaoyin kidney and the Renmai,Tianshu,Yushu,Shenque,Qihai,Guanyuan,Zhongyu and other acupoints.After the included patients were treated at the center,the efficacy was different from the baseline and the total effective rate was 87%after the end of the 5-week treatment.The meta-analysis results of Study 2 show that the combination of acupuncture and medicine for depression is superior to conventional western medicine,and the incidence of adverse reactions is lower than that of conventional western medicine.The clinical research design should be further improved,the research quality should be improved,and a more reliable basis for clinical application should be provided.
