| Background:GERD(Gastroesophageal Reflux Disease)attack rate has reached its peak since 1990 until now.In unite states,the most consumptive use of the drug for the gastrointestinal disorder has been used to GERD.Western medicine has been settled as its standard of GERD in medical science for a long time,and yet it is the only one edenable allopathy.On the other hand,a rate of the edenable symptom has reached only 50%and report that patients suffer from relapse after stop taking a dose.Yang Yun Feng and its team have found that"restricting wood supporting earth" method of treatment is effective in GERD.The research of Zhu Sheng Liang shows that 35%of GERD was in syndrome differentiation of "disharmony of liver and stomach" and 14%was in"deficiency of spleen and stomach",which GERD has closely related to spleen,stomach,liver and gallbladder.In view of several studies,the treatment of GERD by traditional Chinese medicine seems to approach prominent position than western medicine.this research thesis put out the clinical study and META analysis of GERD to prove its excellence of effectiveness of Chinese medicine therapy and introduce to china and abroad countries.the clinical study has processed in observation of progress in GERDQ and SAS in before-treatment,after-4weeks and after-8weeks of using Korean Sa-am Acupuncture and Chinese Herbal Therapy,comparative observation processed based on the results after analysis of curative value.Purpose:META analysis applied in between simple Western medicine therapy and Chinese herbal medicine,simple Western medicine therapy and simple acupuncture treatment and simple Western medicine therapy and Western medicine therapy with acupuncture therapy,thorough its comparative analysis the various results of the thesises of GERD treatment using acupuncture can draw the conclusion of the acupuncture curacy that values of its validity and utility,and also this clinical study of remedy can be another step for GERD treatment to adduce to the world.Furthermore,the comparison between western medicine and Chinese herbal medicine of their curative effectiveness propose the EBM(Evidence-based medicine)to lead the purpose of operability in clinical treatment the clinical study apply the combined treatment of Korean Sa-am acupuncture and Kai Yu He Wei Decoction in adjustment to observe its curacy process and shows the importance of the combined therapy of Chinese herbal medicine and Five transport acupoints.The purpose is based on the provision of information and treatment varieties through the curative assessment of GERD using Chinese herbal medicine and Five transport acupoints.The evaluation of statistical analysis of GERDQ symptoms and SAS can reach the conclusion of curative effect,some problems in the clinical study and the treatment applying Chinese medicine can affect to the medical field in outlook.Method:META analysis research has based on CNKI,VIP,CBM and Wan Fang domestically,and also resource from Pubmed,Cochrane,Embase and Web of Science Database in foreign research.Some of the Korean databases has researched in Kmbase,Kiss,Korean national library,DBpia and RISS.all of the data conduct only RCT thesis and collect the data since the build of the database to January 2017.patients have conducted for the clinical study in the evaluation standard rightly and also apply the GERDQ which is used for clinical evaluation of its curacy to analyse.the data has analysed with Review manager 5.3.the clinical study has conducted 20 GERD patients applying the combined treatment of Korean Sa-am acupuncture and Chinese herbal medicine to observe its clinical curacy.The process of treatment has applied to patients in supine position,sterilize the skin of acupoints before inserting the needle.The manipulation techniques of Sa-am acupuncture apply the method of"supplementing spleen"(supplement spleen and heart and reduce liver)and"reducing stomach"(supplement stomach and pericardium and reduce gallbladder)."supplementing spleen":Shaofu,Dadu on nourishing insertion method and Dadun,Yinbai on reducing insertion method."reducing stomachw:Yang-gu,Jiexi on nourishing insertion method and Zulinqi and Yingu on reducing insertion method.Quick needle insertion with directing the needle tip,the reinforcing method was performed in the same direction of the normal flow along its meridian and the reducing method was performed in the direction against the normal flow of meridian pathway.Retaining needle for 20 mins per treatment,the process of course has 2 times a week,total 16 times of treatment in two-month course was performed.The Chinese herbal medicine is based on syndrome differentiation,apply Kai Yu He Wei Decoction(opening stagnation to harmonise Stomach decoction)in adjustment for treatment.Kai Yu He Wei Decoction includes rhizome of large-headed atractylodes,edible fungus,Pseudostellaria heterophylla,Fructus aurant?,Sepiella maindroni deRoehebrune,Areca catechuL,Hematite,Area subcrenata Lischke,Coptis chinensis Franch,Tetradium ruticarpum,Caulis Perillae and Fritillaria thunbergii.This decoction has prescribed every once of 2weeks in the process.the duration of treatment is same as acupuncture treatment.the data has proceed for comparative analysis with PASW Statistic 18 software.Result:The META analysis of GERD curacy research using acupuncture has summarised with total 23 sentences which correspond to income standard.the collection results of simple WESTERN MEDICINE versus WESTERN MEDICINE plus acupuncture treatment:8 papers in china,1 paper in a foreign country(Netherlands),total 9 articles,every treatment group has conducted 416 cases and 401 cases in control group,total 819 cases.Odds ratio:4.32(95%CI:2.92?6.39,p=0.47).the collection results of simple WESTERN MEDICINE versus Chinese herbal medicine plus acupuncture:7 papers in China.every treatment group has conducted 301 cases,297 cases in control group,total 598 cases.odds ratio:2.94(95%CI:1.61?5.34,p=0.25).the collection result of WESTERN MEDICINE versus acupuncture:7 papers in China,very treatment group has conducted 237 cases,226 cases in control group,the total of 393 cases.combines odds ratio(OR)respectively 3.79(95%CI ? 2.09?6.87,p=0.82).Review manager 5.3 software was used for META analysis,total 3 kinds of analysis have symmetry with funnel plot' s graph shape so that the possibility of bias is comparatively low.The collected sentences are highly safe because the general susceptibility and peculiarity have not found.the clinical study has total 20 cases of GERD patients and its results show below:GERDQ and SAS have decreased gradually during treatment,and the GERDQ comparison between 4th week,8th week and before treatment has statistical differences,p=0.000,p=0.000(p<0.05).the GERDQ comparison between 4th week and the 8th week has statistical differences,p=0.000(p<0.05).the SAS comparison between 4th week and the 8th week has statistical differences p=0.004,p=0.000(p<0.05).the composition of gender was 12 cases of the man and 8 cases of the woman,composed with 60%of man and 40%of woman.the research shows below:the GERDQ analysis of each period the combined therapy of Chinese herbal medicine and acupuncture shows,Lower 8th week of GERDQ score than 4th week and before treatment.it shows statistical differences,t=3.739,p=0.000(p<0.05).the SAS analysis of each period treatment of simple Chinese herbal medicine and the combined therapy of Chinese herbal medicine and acupuncture shows lower results in 4th to 8th weeks of the combined therapy of Chinese herbal medicine and acupuncture.it shows statistical differences,t=2.621,t=2.828,t=3.745 p=0.011,p=0.009,0.001(p<0.05).Conclusion:1.Western medicine therapy plus acupuncture or the combined therapy of Chinese herbal medicine and acupuncture both shows the significant curacy of GERD.2.Western medicine therapy or the combined therapy of Chinese herbal medicine and acupuncture shows better results than simple WESTERN MEDICINE therapy or simple Chinese herbal medicine.3.the combined therapy of Korean Sa-am decrease the rate of GERDQ and SAS of GERD. |
