Androgenetic alopecia(AGA)is the most common type of hair loss at present.It is a hair loss disease that begins in adolescence or late adolescence.It can be caused by both men and women,but hair loss alopecia at the top.Female AGA is mainly characterized by sexual sparse hair on the top of the head and the front hairline.As the crown of tolerance,hair loss has an important impact on the patient's image,self-confidence and quality of life,so hair loss disease is a key disease in dermatology research.In recent years,with the development of society and the change of lifestyle,the incidence of AGA has increased year by year,and there is a trend of aging.There are many treatments for this disease,but any medical treatment method only temporarily relieves the disease.After 1-2 years of cessation of intervention,it gradually returns to the initial state,and can be further aggravated and even bald.Therefore,the prevention and treatment situation of AGA is very serious,requiring joint intervention of multiple treatments,and comprehensive and local comprehensive prevention and treatment is needed.At present,the main means for the rapid onset of AGA treatment are oral finasteride,topical minoxidil and hair transplantation.Effective maintenance methods such as oil control and anti-offwash products and low-energy laser are as important as drug treatment and hair transplant surgery.The traditional Chinese medicine Yufa dandruff lotion has been clinically proven in the previous stage.It is safe and effective in treating AGA,and has good oil control and anti-off effect.In order to further evaluate the efficacy and safety of traditional Chinese medicine medicinal and dandruff lotion,random single blind and autologous The control method was used to observe the hair diameter and quantity of fixed target area,scalp erythema scale and oil secretion evaluation by trichoscopy;and allergic adverse events such as erythema and bullae were recorded,and the safety was evaluated.Through the experimental animal research,the skin color change,terminal hair length and histological changes of the animal model of Chinese medicine hair loss and hair lotion alopecia were observed to verify its curative effect.This paper is divided into two parts:The first part of the literature review:Summary 1 summarizes the new progress of AGA external treatment methods,summed up the treatment characteristics of each method;Summary 2 detailed review of the history of AGA TCM pathogenesis,summed up the new progress of TCM treatment of AGA.The second part of clinical and animal experiments:First,clinical research:Research purposes:combined with clinical and trichoscopy system,comprehensive evaluation of the efficacy and safety of traditional Chinese medicine hair and toilet powder lotion in the treatment of AGA.Methods:The clinical study part adopts random single-blind and autologous control methods.62 patients with AGA were randomly divided into the experimental group(hereinafter referred to as the treatment group)and the blank lotion control group(hereinafter referred to as the control group),including 39 patients in the treatment group and 23 patients in the control group(6 patients in the loss group).The patients in the treatment group were treated with Chinese herbal medicine hair powder and dandruff lotion once a day,each time the amount of lotion was 10g,the time of the lotion in the scalp was not less than 5 minutes,and the control group used blank washing according to the same method.The clinical observation time was 6 months.During the treatment period,the patient was reviewed every 6 weeks.The total hair count,the number of hair shaft diameter variation,the hair shaft diameter variation rate,the scalp inflammation and the improvement of the clinical symptoms of the patients in the CRF table were compared before and after 6 months of treatment.Judging the therapeutic effect of traditional Chinese medicine Yufa dandruff significant differences in the age of onset,duration,and family history between the treatment group and the control group(P>0.05).2.(1)trichoscopy index:There was no difference in the total hair shaft number and hair shaft diameter variation in the left frontal angle of the treatment group(P>0.05),and the left frontal hair shaft diameter variation rate was statistically significant(P<0.05).There was no significant difference in the total number of hair shafts in the treatment group(P>0.05),the number of diameter variation of Baihui hair shaft(P=0.019),and the variation rate of hair shaft diameter were statistically significant(P<0.05);There was no significant difference in the total hair shaft number,hair shaft diameter variation and hair shaft diameter variation rate in the treatment group(P>0.05).There were no significant differences in the total hair shaft number,hair shaft diameter variation and hair shaft diameter variation rate between the left frontal angle,Baihui and hairpin in the control group(P>0.05).The scalp inflammation was significantly different between the treatment group and the control group,and the difference between the treatment group and the control group(P=0.000),which was statistically significant.(2)Clinical subjective symptoms:scalp itching,scalp oil secretion was statistically different(P<0.05),and there was no statistical significance in lotion.The color of the skin of the mice,the length of the hair and the histological characteristics were evaluated to evaluate the Chinese medicine.The effect of lotion on hair growth in mice.Methods:Forty female C57BL/6 mice,6-8 weeks old,were randomly divided into model group,negative control group,positive control group,experimental group and blank lotion group,with 5 groups in total,8 rats in each group.One week after the adaptive feeding,the mice were anesthetized with intraperitoneal injection of pentobarbital sodium,and the back of the mice was depilated by 2 cm4 cm using a depilatory cream.After modeling,the model group did not receive any treatment;the negative control group was given external saline;the positive control group was given topical minoxidil;the experimental group was given the external Chinese medicine hair-curing agent;the blank lotion group was applied to the external matrix.It does not contain traditional Chinese medicine lotion.The dosage of the drug follows the equivalent dose conversion standard between the experimental animal and the human body,and the amount is about 0.4ml/only.The experimental group was smeared with traditional Chinese medicine medicinal hair powder and smear lotion.After evenly rubbing with a cotton swab for 5 minutes,the washing agent was washed with physiological saline.The blank lotion group was used in the same way as the experimental group;the other groups were directly applied with the corresponding drugs,without rubbing.And cleaning.Before taking the medicine every day,take a photo of the back of the mouse under the same light source with a digital camera to observe the changes in skin color and hair growth.On the 9th day after modeling,3 mice were randomly selected from each group,and the back skin tissues were taken.The histological changes were observed under microscope under paraffin section HE staining.On the 18th day after modeling,the remaining skin of each group was taken.After paraffin section HE staining,three fields were randomly taken under the microscope 100 times field of view,the number of hair follicles in each group was observed,and the total number of hair follicles in the field of view was calculated.Finally,the final hair length and back skin color changes were analyzed.Results:1.Skin color change:After 4 to 9 days of application,the skin color of each group changed from pink to gray and finally turned black.The positive control group first began to change skin color,experimental group and blank lotion.The skin color of the group changed later than the other groups.2.Mouse hair length:The average length of the positive control group(6.82±0.14mm)was better than that of the model group(6.45±0.10mm)and the negative control group(6.31±0.13mm),and the positive control group and the model group were compared(<0.05)was statistically significant,compared with the negative control group(P<0.05).The mean length of the experimental group(2.65±0.14mm)and the average length of the blank lotion group(4.08±0.10mm)were higher than other All groups were short,and the differences between the experimental group and the blank lotion group were statistically significant(P<0.05).3.Histological analysis:After HE staining,the number of hair follicles was counted 100 times under the microscope,and the average counts of hair follicles in the positive control group(59.33±9.20)and the treatment group(57.6±3.93)were not statistically different(P>0.05)and better than the model group(35.33±5.23),the negative control group(31.33+5.24),the blank lotion group(44.33±3.84),the positive control group and the experimental group were compared with the model group and the negative control group,respectively.The differences in the blank lotion group were statistically significant(P<0.05).There were no significant differences between the model group,the negative control group and the blank lotion group(P>0.05).Conclusion:In the mouse experiment,the traditional Chinese medicine hair styling lotion is ineffective in skin color change and hair length in mice;it is effective in promoting the early entry of mouse hair follicle into the growth phase.
