Correlation Study Between Expected Value And Acupuncture In The Treatment Of Chronic Tension-type Headache And Acupuncture Sensation

ObjectiveIn this study,patients with chronic tension-type headache(CTTH)were studied.Acupuncture treatment was used as an intervention method,and MASS scale was used as a real acupuncture assessment method,in order to observe the the correlation between the expectant value of the therapeutic effect and the clinical efficacy before treatment and the correlation between the needling expectation and the true needle sense were observed.Methods110 patients with CTTH who meet the criteria were enrolled.The patients were randomly divided into the deep puncture group and shallow puncture group by the Brightech Clinical Information Management System(CIMS)system,55 cases in each group.The treatment selected Baihui,bilateral Fengchi,Taiyang,Hegu and Taichong as acupuncture points.Deep penetration group took deep penetration,the range of needle penetration was between 3 and 5mm,the needle retention time was 30 min,and the total manipulating needle time during needle retention was two.Shallow thorn group to take shallow thorns,needle needle amplitude 1 ~ 3mm,needle retention time was 30 min and manipulating needle was not performed.The treatment period was 8 weeks,in which the first 4 weeks were treated 3 times a week and the 4 weeks were treated twice a week.Each patient had a total of 20 treatments.Prior to acupuncture treatment,item 2 of the Acupuncture Expectation Evaluation Table was used to evaluate the expectation of efficacy in both groups of patients.Item 2 is entitled “What do you think is the effect of acupuncture on chronic tension-type headaches?” The answers are: 1 The effect is very good(the symptom is improved by more than 75%);2 The effect is generally(the symptom is improved by 50% to 75%);3 The effect is slight(the improvement of the symptom is 25% to 50%);4 There is no change(the symptom is improved by 0% ~25%).Those whose symptoms improved the by 50% or more were treated as high-expectant groups,and those whose symptoms were improved by 50% or less were treated as low-efficacy groups.At 8 weeks of treatment,the clinical efficacy indicators of headache between the high-expectant group and the low-expectant group were compared,namely,the number of days of headache attack,Visual Analogue Score(VAS),and duration of headache(h)to observe the effect of the expected value of acupuncture treatment on CTTH patients.Prior to acupuncture treatment,item 4 of the Acupuncture Expectation Evaluation Table was used to evaluate the acupuncture expectancy values of the two groups of patients.The title of Item 4 is: "What kind of acupuncture strength do you hope to achieve during acupuncture treatment?" By measuring the desired acupuncture strength value(the highest point is 10 points and the lowest score is 0 points in theory)in the treatment of the acupuncture marked on the scale with a length of 10 cm,the patient's expected acupuncture sensation value can be obtained.The actual acupuncture sensation in CTTH patients was evaluated by the MASS scale,and the correlation between the acupuncture expectation value and the actual acupuncture score was analyzed to observe whether there was a correlation between the two.Finally,the correlation between the acupuncture expectation value and the patient's baseline period variables(such as height,weight,duration,degree of headache,headache time,headache days,etc.)was analyzed to observe the influencing factors of the acupuncture expectancy value.Results1.The study brings into 110 cases of chronic tension type headache,of which 20 cases are dropped and 90 cases of CTTH patients are included in the statistical analysis.In 90 cases,76 cases belongs to high expectation group,24 cases belongs to low expectation group.2.Baseline data analysis results: there is no statistical difference between the two groups of patients' age,sex,course of disease,anxiety and depression score,headache days and headache time.(p<0.05)3.Results of the evaluation of the expectation of acupuncture and moxibustion: There were statistically significant differences between Eighth groups(P < 0.05)“In your impression,what is the curative effect of acupuncture and moxibustion for treating diseases?” The study finds that the expectation value of patients with bad impression of acupuncture treatment is 0.851 times the expected value of patients who have a good impression on acupuncture and moxibustion treatment(P < 0.05)through the GLM generalized linear model.4.Comparison results of high and low expectant group: At 8 weeks,there is no significant difference in headache,headache and headache duration between the two groups(P > 0.05),and there is no significant difference between the two groups(P > 0.05).5.The correlation between needle aspiration and real needle sensation shows that there is a positive correlation(r=0.298,P < 0.05).The correlation analysis between the expectant value of needle sensation and the patient's baseline variables(such as height,weight,course of disease,headache,headache time,headache days)suggests that there is a correlation between the expectation value of the needle and the time of headache and headache(the degree of headache r=0.218,P < 0.05;headache time r=0.276,P < 0.05).Through the GLM generalized linear model,it is found that the needle sense expectation is only statistically different from the patient's headache time(P < 0.05).The headache time is increased by 1 points,and the needle sense expectancy is increased by 0.072,but it was not related to the degree of headache(P > 0.05).Conclusion1.The expected value of curative effect is not related to the clinical efficacy of acupuncture for the treatment of CTTH.2.The better the impression of acupuncture treatment is,the higher the expected value of CTTH is.3.The expected value of needle sensation is positively correlated with the real needle sensation,that is,the higher the needle sensation expectation is,the stronger the real sense of acupuncture is.There was a positive correlation between the expected value of acupuncture sensation and the time of headache,that is,the longer the headache time,the higher the expected value of needle sensation.
