| Objective Through the application of traditional Chinese medicine fire therapy and simple fire therapy to treat patients with cold lumbar muscle strain,compare the clinical efficacy of the two methods.Methods All the 60 patients were from the Department of Orthopaedics and Traumatology at a top-notch hospital in Hunan Province from August 2016 to October 2017.Patients with lumbar muscle strains that meet the diagnostic criteria were randomly divided into experimental and control groups.Each group had 30 cases.The experimental group used Duhuo parasitic soup wet compress combined with fire therapy;the control group was pure fire therapy.The number of interventions was 20,once a day for 30 minutes.Observed two groups of patients before and after treatment in the VAS score,Oswestry disability index(ODI)score,clinical symptom integral and and plasma TXB2,6-K-PGI1 a levels were measured in both groups before and after treatment.SPSS20.0 software was used for statistical analysis.Descriptive statistics were used for general datum,and(`x±s)was used for measurement data.The observation datum of normality were observed.Independent sample t-test was used between groups and paired before and after comparison.The t-test,which does not meet the normal distribution,adopts the rank sum test;the count datum were compared with the chi-square test,and the rank sum test is not used according to the chi-square test conditions;the rank datum were used the rank sum test.The difference was considered statistically significant(P<0.05).Results1.Comparison of VAS,ODI and clinical signs:the scores of VAS before and after intervention in the experimental group were 5.60±1.16 and 1.47±0.51(t=19.918;P<0.01)and ODI scores were 22.52±9.45 and 8.59±5.04(t=13.486;P<0.01)and clinical signs scores were 10.47±1.85 and5.07±1.66(t=21.438;P<0.01).The scores after the intervention were all lower than those before the intervention.The VAS scores before and after the intervention in the control group were 5.63±1.10,2.47±0.51(t=17.006;P<0.01),and the ODI scores were 21.26±7.60,12.30±4.51(t=14.085;P<0.01),and the clinical signs score was 10.57±1.68.The scores of6.13±1.43(t=20.841;P<0.01)were all statistically significant.The scores after the intervention were lower than those before the intervention.2.The clinical efficacy of the two groups was compared: the experimental group recovered 0 cases,markedly effective in 7 cases(23.3%),effective in23 cases(76.7%),and ineffective in 0 cases;the group was cured 0,markedly effective in 3 cases(10%),effective in 26 cases(86.7%),and ineffective in 1case(3.3%).The total effective rate of the two groups was higher than that of the treatment group(Z=-2.971,P<0.01).Rank sum test showed that the average rank of the trial group was 37.18,which was significantly higher than the average rank of the control group(23.82).The experimental group was superior to the control group.3.The comparison of TXB2 and 6-KGF1 a levels in the experimental group:The TXB2 levels was statistically different before and after treatment(P<0.01),and the content of 6-K-PGF1 a was significantly different(P<0.01).TXB2 levels were lower than before treatment,and 6-K-PGF1 a was higher than before.Conclusion1.The combination of Duhuo Jisheng decoction combined with fire therapy and simple fire therapy can reduce the VAS,ODI and clinical symptom scores in patients with cold-waisted lumbar muscle strain.However,the effect of combination of Duhuo Jisheng decoction and fire therapy is better than that of pure fire therapy.2.The combination of Duhuo Jisheng decoction with fire therapy could reduce the level of TXB2,increase the level of 6-Keto-PGF1 a,decrease TXB2 in peripheral blood,inhibit platelet aggregation,prevent thrombosis,prevent vasoconstriction or spasm,and reduce the occurrence of pain. |
