| Objective:To evaluate the effect of arginine enhancement in early enteral nutrition on nutrition, immune index, gastrointestinal adverse events, vital signs, in-hospital days and other responses in the postoperative gastric cancer patients.Study the reasonable use time of arginine for the postoperative gastric cancer patients to provide feasible nutrition treatment.Method:This is a randomized,controlled trial.40cases of patients with the postoperative gastric cancer patients were randomly divided into study group and control group, each group has20cases.Two groups patients were tested in the14day after surgery. The daily nutritional goal was25kcal/(kg·d).study group is tested by arginine enhanced on the basis supplemented. The nutritional therapy before1days, for8days,15day of early morning, fasting venous blood from patients, detection of nutritional index,Immune index, and the observation of vital signs, gastrointestinal adverse reactions, in-hospital days and other indicators.Results:1. Two groups of patients in nutrition therapy has no death and severe complications such as liver and kidney heart failure, signs of life is not significantly changed. Between the two groups of patients with adverse reactions has no significant difference (P>0.05).we can confirm that arginine enriched enteral nutrition for postoperative patients with gastric cancer is a safe and effective nutrition treatment.2Nutrition index:two groups of patients in nutrition therapy for8days,15days compared with before treatment, nutritional indexes were more significantly elevated. Treatment for fifteenth days and eighth days: Patients with albumin, total protein levels increased significantly(P<0.05). And the test group was higher than that of the control group(P<0.05). Description of arginine enriched enteral nutrition in postoperative patients with gastric cancer can improve the nutrition index, and nutrition therapy lasted14days is better.3.Immune index:two groups of patients in nutrition therapy for eighth days,15days compared with before treatment, immune indices were significantly elevated(P<0.05). Treatment for fifteenth days and8days: two groups of patients of CD4+、CD8+、CD4+/CD8+level rises significantly(P<0.05). The level of IgA in test group was better than the control group. To show that arginine enriched enteral nutrition in postoperative patients with gastric cancer can improve the immunity and nutrition intervention lasted14days, the effect is more obvious, its effect is better than the routine enteral nutrition.4. In-hospital days:test group of patients with postoperative hospital stay for(12.8±3.4)d, Significantly lower than the control group(16.9±6.4)d(P<0.05).we can Confirm that arginine used in postoperative patients with gastric cancer can shorten the time of hospitalization.Conclusions:1.In the postoperative gastric cancer patients with early supplement, the use of arginine is safe and feasible, with no signs of life change obviously, lower incidence of gastrointestinal adverse reaction.2. Arginine enriched enteral nutrition in postoperative gastric cancer patients with in can enhance the immune function of organism3. Arginine enriched enteral nutrition in postoperative gastric cancer patients can improve nutritional status, correcting negative nitrogen balance.4. Arginine enriched enteral nutrition in postoperative gastric cancer patients can shorten the time of hospitalization, is a kind of economic and effective treatment method.5. Arginine enriched enteral nutrition in postoperative gastric cancer patients, nutritional therapy lasted14days, immune and nutrition indexes, effect is more conspicuous. |
