Influences Of Preoperative Enteral Nutrition Combined With Probiotics On The Clinical Outcomes In Postoperative Gastric Cancer Patients

Objective: The aim in this study is to investigate the effect of micro ecological enteral nutrition on the clinical outcomes of patients undergoing radical gastrectomy.Methods: Prospectively included 60 patients with gastric cancer who were admitted to our hospital from February to October in 2014,randomly divided into 3 groups,that is,conventional treatment group(CONT group),normal enteral nutrition group(NEN group)and micro ecological enteral nutrition group(MEEN group),20 cases respectively.Patients in group MEEN were adopted micro ecological enteral nutrition before operation,the patients in group NEN were rendered enteral nutrition,and the two groups were received continuous enteral nutrition for 7 days;the patients in group CONT were offered routine treatment.The rate of infectious complications and other complications,postoperative anal exhaust time and postoperative hospital stay were observed.The indexes of inflammatory response,nutrition and body fluid immunity were detected in the day before operation,the third day and the seventh day after operation.Stools were detected for target intestinal bacteria on the first day before surgery and postoperative first natural defecation.Results: The rate of postoperative infectious complications in group MEEN was lower than in group CONT(3.3% VS 20%,p<0.05).There was no significant difference in the rate of postoperative complications(postoperative anastomotic fistula,intestinal obstruction,gastroparesis syndrome)among three groups(p>0.05).For the first exhaust time of anus after operation,the MEEN group and the NEN group were earlier than the CONT group(p<0.05),but there was no significant difference between the MEEN group and the NEN group(p>0.05).The postoperative hospital stay in MEEN group and NEN group were less than group CONT(p<0.05),but there was no statistically significant difference between MEEN group and NEN group differences(p>0.05).There was no significant difference in the level of inflammatory response at first day before operation and seventh day after operation(p>0.05),the levels of inflammatory response in patients with MEEN group were lower than those in group NEN and group CONT at the third day after operation(p< 0.05).The level of nutritional index was no significant difference among three groups at first day before operation and third day after operation(p>0.05);On seventh day after operation,the level of nutritional index of group MEEN was higher than that in group NEN and group CONT(p<0.05),and there was no significant difference between NEN group and CONT group(p>0.05).In the level of humoral immune index,there was no significant difference among three groups in the first day before operation(p>0.05);On third day and seventh day after surgery,there was no significant difference in the level of Ig M among three groups(p>0.05),the levels of Ig G and Ig A were higher in group MEEN than in group NEN and group CONT(p<0.05),furthermore there was no significant difference between the NEN group and the CONT group(p>0.05).Comparison of intestinal target bacteria counting was no significant difference among three groups on the preoperative first day(p>0.05);After first time of spontaneous defecation,EB and EC count of MEEN group were lower than NEN group and CONT group,BL and LC count were higher than the NEN group and CONT group,the difference was statistically significant(p<0.05),and that there was no significant difference between the NEN group and the CONT group(p>0.05).Conclusion: According to the nutritional risk of patients undergoing radical gastrectomy,the use of enteral microecopharmaceutics nutrition is beneficial to reduce the early postoperative inflammatory reaction,promote the recovery of nutritional status and immune function,and to correct the imbalance of intestinal flora.So it can effectively reduce the incidence of postoperative infectious complications,shorten the postoperative hospital stay,and promote the rapid recovery of patients.
