Comparative Study On Early Postoperative Enteral Nutrition After Gastric Cancer Surgery

Objective:Nowadays,With the development of clinical nutrition,the enteral nutrition support treatment in the postoperative therapy is attracting more and more attention.according to the statistics,In all the tumors in China,The incidence of gastric cancer was the second highest,the surgical resection is still the main treatment.Because of the consumption of gastric cancer and the loss of operation to the body,the nutritional status of patients after operation is often unsatisfactory,which further affects the recovery of patients after operation.Our enteral nutrition for patients with complex classification,the effect of different enteral nutrition of patients with gastric carcinoma is still a lack of comparison,this research aims at a comparative study of two clinical application of enteral nutrition(Short Peptide Enteral Nutrition Powder and Intacted Protein Enteral Nutrition Powder)is applied to the clinical efficacy of gastric cancer after surgery.Methods:Totally 90 operative patients with gastric cancer from October 2015 to December 2016 in our hospital were randomly divided into Short peptide group(n =45)and Intacted protein group(n =45).Each group received nutritional support of the same calorie from the 2nd day of postoperation.On the day before operation?the 1st day after operation and the 8th day after operation,the concentration of albumin(ALB)?prealbumin(PA)?haemoglobin(HGB)and lymphocyte count(LYC)were measured,record the time of anal exhaust,meanwhile observe and record of follow index:the incidence of complication and enteral nutrition related adverse reaction.Using the Statistical software SPSS 19.9 for data analysis,P<0.05 presents significant difference.Results:1.Blood indexThe blood indicators(ALB,PA,HGB,LYC)in both groups on 8th day were significantly improved compared to those at day one after operation(both P<0.05).The improvements of the indications in the short peptide group were not significant,compared with those of the whole protein group(P>0.05)2.Enteral nutrition related adverse reactions:In the short peptide group:6 cases(13.6%)had diarrhea,4 cases(9.1%)had abdominal distension,3 cases(6.8%)had nausea and vomiting.In the whole peptide group:14 cases(31.1%)had diarrhea,13 cases(28.9%)had abdominal distension,2 cases(4.4%)had nausea and vomiting.The incidence of diarrhea in the short peptide group was significantly lower than that in the whole protein group(P<0.05)The incidence of abdominal distension in the short peptide group was significantly lower than that in the whole protein group(P<0.05)There was no significant difference in the incidence of nausea and vomiting between the two groups(P>0.05)3.Postoperative complicationsThere were 7 cases(15.9%)had postoperative complications in the short peptide group,and there were 9 cases(20%)had postoperative complications in the whole protein group.There was no significant difference in the incidence of complications between the two groups(P>0.05).4.Gastrointestinal function recovery:The exhaust time from anal of the short peptide group was 52.4±6.1h,The exhaust time from anal of the whole protein group was 52.7±6.4h,there was no significant difference between the groups(P>0.05).5.The cost of ENThe average cost of nutrient support in short peptide group was 2216 ± 137Yuan,the average cost of the whole protein group was 1771 ± 111 Yuan,and the correlation between groups was significant(P<0.05).Conclusion:1.Both of the short peptide enteral nutrition(Baipuli)and the whole protein enteral nutrition(Nengquanli)can significantly improve the nutritional status of gastric cancer patients after operation.2.Early use of the short peptide enteral nutrition is better tolerated than the whole protein enteral nutrition for gastric cancer patients after operation.3.There was no significant difference in the incidence of postoperative complications and resumptions of gastrointestinal function between the two groups,considering that the short peptide enteral nutrition is almost about twice as expensive as the whole protein group,for postoperative patients with gastric cancer can use short peptide enteral nutrition at the first time,then gradually changed to whole protein enteral nutrition after the recovery of gastrointestinal function.
