As one of the professional methods of social work,case management plays an important role in many practical fields.This study attempts to use the case management concept and professional methods to intervene in multiple dilemmas of two special patients with cerebral palsy and bipolar disorder,and implement case management of dual-serving subjects.In this process,cross-design the mutual support services between special patients for the service needs of the two,and explore the implementation process and content of the double-blind case for special patients with the support and assistance of the case management method,and The role of different service methods in this intervention process and implementation effectiveness,forming a complete service process from requirements assessment,service design design implementation to post-assessment.The introduction of double-blind cases is a preliminary attempt to solve the practical problems from the characteristics of dual-service objects.Through the evaluation of the psychological and behavior aspects of the service objects in the early and late stages,this method can alleviate the service objects within the scope of ensuring security and control.The problem and help the service object grow.In this paper,the service object small S and small Z demand meet the two main lines to carry out the service,case management as a method of assistance and support,respectively,the service object small S and small Z provide double-blind management services,within the overall service system Coordinate the mobilization of relevant resources,support the creation of double-blind case service scenarios,meet the multiple needs of the client,and ensure the smooth implementation of double-blind cases.In this process,the service objects are supporters of each other,benefiting from the activities of actually participating in helping each other,satisfying the needs of the other party,solving their own problems as much as possible,improving their own capabilities,and realizing the two-way growth of the service objects.Through case management and double-blind case service,we can see more possibilities and advantages potential of special patients.The service target small Z community education is basically realized,the basic knowledge of language and mathematics computing ability are enhanced,and the community age support system is gradually established.Increased participation in activities,fostered active learning habits,improved learning awareness,increased self-efficacy,and integration with post-delivery services.Small S participates in the goal of community activities,and improves the bad behaviors such as excessive drinking water,emotional changes,and excessive dependence.By participating in community volunteer teaching activities,self-positive evaluation increases and life self-confidence improves. |
