Clinical Study On The Efficacy And Safety Of Hui Yang Yu Long Decoction In Treating Chronic Wound Pain And Healing Of Yang Deficiency And Cold Coagulation Syndrome

Objective:To observe and evaluate the efficacy and safety of Hui yang yu long decoction on the improvement of pain in chronic wound debridement period of Yang deficiency cold coagulation syndrome and its promoting healing effect.Methods:Random single blind parallel controlled study was used.all medical records were collected in the sore and peripheral vascular surgery ward of the first affiliated hospital of Tianjin university of traditional chinese medicine.90 patients with chronic wound with yang deficiency cold coagulation syndrome were randomly divided into 3 groups.blank group(n=30)was treated with saline intervention,intervention with lidocaine injection in intervention group 1(n=30)and intervention with Hui yang yu long decoction in intervention group 2(n=30).The wound pain score,local TCM symptom score,wound area and other indexes were observed and compared in the three groups,and the observation period was 2weeks.Results:The baseline data of the three groups were not statistically significant and could be compared.(1)Comparison of pain between 1h before dressing change and dressing change between 3 groups after treatment(p<0.05),and the blank group(2.683±0.9279)>intervention group 1(2.467±0.9409)and intervention group 2(2.050±0.9129).There was significant difference in pain contrast between the same time node in 3 groups after treatment(p<0.0001),and the blank group(5.88±1.71)>intervention group 2(1.73±1.41)and intervention group1(0.93±1.81)(take day 14 as an example).(2)Local TCM syndrome score:There were significant differences in the local TCM syndrome score between the three groups after treatment(p<0.0001),and the blank group(39.03±9.626)>intervention group1(34.83±11.15)>intervention group 2(21.40±10.58).After treatment,there were differences in the evaluation of the curative effect of the three groups(p<0.05),and the blank group was ineffective in 13 cases,effective in 17 cases;the intervention group 1 was ineffective in 6cases,effective in 21 cases,effective in 3 cases;the intervention group 2 was ineffective in 2cases,effective in 21 cases,effective in 6 cases,clinical in 6 cases,1 case was cured.(3)wound area:there were significant differences in wound area between the three groups after treatment(p<0.0001),and the blank group(12.87±16.85)cm~2>intervention group1(8.577±6.862)cm~2>intervention group 2(3.641±7.194)cm~2.After treatment,there were differences between the three groups(p<0.05),and the blank group was ineffective in 11cases,effective in 18 cases,effective in 1 case;intervention group 1 ineffective in 8 cases,effective in 17 cases,effective in 5 cases;intervention group 2 ineffective in 3 cases,effective in 21 cases,effective in 5 cases,clinical recovery in 1 case.(4)Anxiety score:3 after treatment There were differences in anxiety scores between groups(p<0.05),and in the blank group(44.80±9.053)>intervention group 1(39.43±7.960)>intervention group 2(35.77).(5)Safety evaluation:during the course of the experiment,90 subjects did not have local side effects and adverse reactions or other adverse reactions associated with the test external drug,as well as abnormal laboratory indicators.Conclusion:Hui yang yu long decoction has the effect of warming yang and relieving coagulation and activating blood circulation to relieve pain.It can produce analgesic and promoting healing effects for chronic wounds with Yang deficiency and cold coagulation syndrome.Considering that the treatment effect of Hui yang yu long decoction is better than lidocaine injection,it is safe in clinical use and has the advantage of pharmacoeconomics.
