Objective :To observe the clinical effect of "Tiaoshen Jiangni " acupuncture on patients with hiccup after stroke.Methods:68 patients with post-stroke hiccup who were admitted to the special needs department of acupuncture and moxibustion in the first affiliated hospital of tianjin university of traditional Chinese medicine were collected,all cases met the inclusion criteria of this study,the patients were divided into treatment group(Tiaoshen Jiangni acupuncture group)and control group(ordinary acupuncture group)by random number table method,there were34 cases in each group.Due to the age discrepancy of the included patients,1 case was excluded from the Tiaoshen Jiangni acupuncture group and 2 cases from the ordinary acupuncture group,33 cases were actually included in the Tiaoshen Jiangni acupuncture group and 32 cases in the ordinary acupuncture group.The patients in both groups were treated on the basis of stroke.the Tiaoshen Jiangni acupuncture group was treated with the method of Tiaoshen Jiangni acupuncture,acupoint selection: Neiguan(double),Renzhong,Fengchi(double),Wangu(double),Yifeng(double),Tiantu,Zusanli(double).the ordinary acupuncture group used ordinary acupuncture method,acupoint selection: Geshu(double),Neiguan(double),Zhongwan,zusanli(double),Danzhong.Needles were retained for 20 minutes in both groups,once a day for 5 days.After the treatment,the relevant indicators were evaluated,two groups of patients before treatment,treatment 3 day,5 days to observe the episodes of hiccups,time interval,symptom and Pittsburgh sleep quality index scale(PSQI)score,Hamilton anxiety scale(HAMA)score,geriatric depression scale(GDS)score,to evaluate the recovery rate and total response rate,duration and duration of treatment,sleep,anxiety,and depression of the two groups of different treatment methods,and to compare the changes in laboratory data and adverse reactions before and after treatment,and observe the safety of the two groups of treatment regimens.Results:1.General data comparison before treatment,this study participants included 68 cases of patients,3 cases were excluded,and finally 65 cases were included.There were 33 cases in the Tiaoshen Jiangni acupuncture group(34 cases included,1 case excluded)and 32 cases in the ordinary acupuncture group(34 cases included,2 cases excluded).There was no significant difference between the two groups in gender,age,stroke type,stroke incidence,severity of hiccup symptoms before treatment,PSQI score,HAMA score,GDS score and other indicators at baseline(P>0.05),suggesting comparability.2.Main efficacy indicators2.1 Total response rate and recovery rate: The total effective rate of the Tiaoshen Jiangni acupuncture group was 93.9% and the recovery rate was 63.6%;the total effective rate of the ordinary acupuncture group was 87.5% and the recovery rate was 28.1%.These results indicated that the two treatment regiments were effective for patients with post-stroke hiccup,but the treatment regiments in the Tiaoshen Jiangni acupuncture group were superior to those in the ordinary acupuncture group(P<0.05).2.2 Onset time: In the Tiaoshen Jiangni acupuncture group(67.3%)and the ordinary acupuncture group(28.1%),the onset time was significantly earlier than that of the ordinary acupuncture group(P<0.05).2.3 Course of treatment: In the Tiaoshen Jiangni acupuncture group,11 cases(33.3%)were cured within 3 days;a total of 16 cases(48.5%)were cured within 4-5 days.;6 cases(18.2%)were cured after more than 5 days;in the ordinary acupuncture group,5 cases(15.6%)were cured within 3 days;a total of 8 cases(25%)were cured within 4-5 days;a total of 19 cases(59.4%)were cured after more than 5 days.The course of treatment in the Tiaoshen Jiangni acupuncture group was significantly shorter than that in theordinary acupuncture group(P<0.05).3.Secondary efficacy index3.1 Pittsburgh sleep quality index scale(PSQI)score: Comparison within the two groups:PSQI scores of patients in the two groups were significantly improved on the fifth day of treatment(P<0.05),but no significant improvement was observed on the third day(P>0.05);Comparison between groups: on the third day of treatment,there was no significant difference in PSQI scores between the Tiaoshen Jiangni acupuncture group and the ordinary acupuncture group(P>0.05),but there was a significant difference on the fifth day of treatment(P<0.05).It indicated that both groups could effectively improve the sleep quality of patients at 5 days of treatment,while no significant improvement was observed on the 3rd day of treatment(P>0.05);there was no significant difference between the two groups in the effect of sleep improvement on the 3rd day of treatment,while the Tiaoshen Jiangni acupuncture group was significantly better than the ordinary acupuncture group on the 5th day of treatment.3.2 Hamilton anxiety scale(HAMA)score: Comparison within the two groups: HAMA score on day 3 and day 5 of treatment in both groups significantly improved(P<0.05);Comparison between groups: HAMA scores of the two groups showed no significant difference on the third day of treatment(P>0.05),but significant difference on the fifth day(P<0.05).The results showed that the two groups could effectively improve the depression of patients on the third and fifth days of treatment,and the two groups had no significant difference in the effect of sleep improvement on the third day of treatment,while the Tiaoshen Jiangni acupuncture group was significantly better than the ordinary acupuncture group on the fifth day of treatment.3.3 Geriatric depression scale(GDS)score: Comparison within the two groups:HAMA score of both groups did not significantly improve on the third day of treatment(P>0.05),and significantly improved on the fifth day of treatment(P<0.05);comparison between the two groups: HAMA scores on the third and fifth days of treatment showed no significant difference between the two groups(P>0.05).The results showed that both groups could effectively improve the depression of patients on the 3rd and 5th day of treatment,but there was no significant difference between the two groups.Conclusion:1.The Tiaoshen Jiangni acupuncture method and the ordinary acupuncture method can improve the patients' hiccup symptoms,and the effective rate and recovery rate were better than the ordinary acupuncture method.2.The effect time and course of treatment of Tiaoshen Jiangni acupuncture to treat the hiccup after stroke were shorter than that of the ordinary acupuncture method.3.Both the Tiaoshen Jiangni acupuncture method and the ordinary acupuncture method can effectively improve the sleep and mood of patients(anxiety,depression),and in terms of improving sleep and anxiety,the Tiaoshen Jiangni acupuncture method is significantly better than the ordinary acupuncture method.4.The treatment of post-apoplexy hiccup was simple,convenient,economical and effective with the method of Tiaoshen Jiangni acupuncture method and ordinary acupuncture method,the Tiaoshen Jiangni acupuncture method is better than the ordinary acupuncture method,which is more worthy of clinical application and widespread promotion.
