| Objective:Atopic dermatitis in children is a common pediatric disease,and the incidence of it has gradually increased in recent years.In particular,children with moderate or severe skin itching,disfigurement and long-term medication brought psychological and mental stress,seriously affect the children’s learning,life and growth.Traditional Chinese medicine through syndrome differentiation and treatment,the overall conditioning of children’s physique,in the improvement of the state of skin lesions,clinical symptoms and reduce the long-term recurrence rate is effective.This paper studies the clinical efficacy of Qingxin Xiepi recipe in the treatment of children with atopic dermatitis(heat accumulation of heart and spleen),and verifies that it can improve the skin lesions of moderate and severe atopic dermatitis,TCM syndromes and reduce long-term recurrence rate and other aspects of clinical efficacy,and compared with traditional western medicine treatment(external glucocorticoid)efficacy,to explore the combination of Chinese medicine treatment of children atopic dermatitis feasibility and advantages and disadvantages,to provide reliable evidence for clinical application.Methods:In this study,69 children with moderate and severe atopic dermatitis(heart and spleen heat accumulation syndrome)were selected.According to the method of random and parallel control in stratified group,the children were divided into 2 groups,experimental group and control group.The experimental group used oral traditional Chinese medicine decoction "Qingxin Xie spleen prescription" combined with external use of fluticasone propionate cream,control group external use of fluticasone propionate cream,both groups used "Chinese children atopic dermatitis diagnosis and treatment consensus" health education,clean bath and other basic treatment.2 weeks as a course of treatment,a total of 2 courses of treatment.Pre-treatment,2 weeks of treatment,4 weeks of treatment,4 weeks after withdrawal and 12 weeks after withdrawal The symptoms and signs were SCORAD and TCM syndromes,and SPSS22.0 statistical software was used for statistical analysis.Results:A total of 69 cases were collected in this study,9 cases fell off,among which 4cases fell off in the experimental group and 5 cases in the control group,and 60 cases werefinally included in the statistical case,and 30 cases were each in the experimental group and the control group.Both groups had no statistical difference in general condition,pre-treatment history,SCORAD score and TCM syndrome score,which was comparable.(1)After treatment,the effective rate of the experimental group was 100%,the recovery rate was 70%,and the control group was 90%,the recovery rate was 16.7%.Both groups had obvious curative effect,but the difference between the two groups was statistically significant(P<0.01),which indicated that oral Chinese medicine decoction Qingxin Xiepi prescription combined with external use of fluticasone propionate cream was better than the treatment of fluticasone propionate cream alone.(2)The scores of the experimental group and the control group SCORAD treated for 2weeks and 4 weeks were significantly decreased,and the difference was statistically significant(P<0.01),indicating that both groups had obvious curative effect on relieving the clinical symptoms and signs of the children.The SCORAD score of the treatment group was lower than that of the control group at 4 weeks of treatment,and the difference was statistically significant(P<0.01),suggesting that the effect of the experimental group was better than that of the control group at the later stage of disease treatment.(3)Both the experimental group and the control group were treated for 2 weeks and treated for 4 weeks,and the total score of TCM symptoms was significantly,and the difference was statistically significant(P<0.01),indicating that both groups had obvious curative effect on improving the overall TCM symptoms of children.Compared between groups,the total score of TCM symptoms in the treatment group was significantly lower than that in the control group at 2 weeks of treatment and 4 weeks of treatment,and the difference was statistically significant(P<0.01),suggesting that the curative effect of improving the overall symptoms of TCM in the whole treatment group was better than that in the control group.(4)The scores of Chinese medicine symptoms for 2 weeks and 4 weeks of treatment were significantly reduced,and the difference was statistically significant(P < 0.01),suggesting that the experimental group had significant efficacy in improving the symptoms of Chinese medicine.Control group treatment for 2 weeks,treatment of 4 weeks itching,sleep disorder,upset irritability 0,thirst and drink,urinating short red symptoms points significantlyreduced,the difference is statistically significant(P 0.05),there was no significant change in the symptoms of stool dry knots,The difference was not statistically significant(P<0.05),suggesting that the control group had no obvious effect on improving itching,sleep disorder,irritability,thirst and drinking,and urinating short redness,and no obvious effect on alleviating the symptoms of stool dry knots.Intergroup comparison,treatment 2 weeks experimental group itching,sleep disorder,thirst drink,urinating short red,stool dry knot symptoms integration is lower than the control group,the difference is statistically significant(P>0.05),suggesting that the experimental group treatment 2 weeks to improve the above 5Chinese medicine symptoms on the efficacy is better than the control group;The difference was statistically significant(P < 0.05),indicating that the treatment was 4 weeks,and the experimental group was better at improving the symptoms of the above-mentioned 6 Chinese medicine.(5)The SCORAD score and the total score of TCM symptoms were significantly lower in the two groups(P<0.05).The difference was statistically significant improving the symptoms of itching,sleep disorder,irritability,thirst and drink,short red urine and dry stool.Comparison between groups,the experimental group in SCORAD points,TCM symptoms All of the symptoms and symptoms of TCM were lower than those of the control group,and the difference was statistically significant(P<0.05),which indicated that the experimental group was better than the control group in terms of long-term curative effect and reducing recurrence rate.(6)The relapse rate of the experimental group was 10% and that of the control group was 33.3% after 4 weeks of drug withdrawal,the difference was statistically significant(P <0.05);the relapse rate of the experimental group was 16.6% and that of the control group was43.3% after 12 weeks of drug withdrawal,the difference was statistically significant(P <0.05).In conclusion,the experimental group is better than the control group in reducing the long-term recurrence rate.Conclusions:(1)Oral traditional Chinese medicine decoction Qingxin Xiepi prescription combined with external use of fluticasone propionate cream treatment of children atopic dermatitis is effective.The effect of internal traditional Chinese medicine decoction combined withexternal use of glucocorticoid in the treatment of moderate and severe atopic dermatitis in children is better than that of external use alone.(2)Oral Chinese medicine decoction Qingxin Xiepi prescription combined with topical fluticasone propionate cream is more lasting,effectively reduce the recurrence rate of disease,relieve the overall symptoms of children,improve physique,better prognosis.(3)The Chinese medicine in the treatment of children atopic dermatitis,relief symptoms,safe and reliable,and avoid long-term external use of hormones and other western medicine brought side effects,more easily accepted by children and parents,fully reflects the advantages and characteristics of traditional Chinese medicine.enriched the dominant diseases in pediatrics,and provided a new treatment method for the clinical treatment of moderate and severe atopic dermatitis in children,which is worthy of clinical popularization and application. |
