Atopic dermatitis,also known as atopic eczema,is a common inflammatory skin disease in dermatology.The studies on atopic dermatitis in traditional Chinese medicine and Western modern medicine have been carried out one after another.There are many treatments.However,the exact pathogenesis and effective means of atopic dermatitis have not yet been identified.Therefore,under the guidance of traditional Chinese medicine theory,combined with the research results of modern medicine,it has been the focus of dermatologists to explore the treatment methods that can improve the clinical symptoms of AD,reduce the recurrence,and increase medical safety.Objective:From the view of pathogenesis of "spleen deficiency and heart fire",the clinical efficacy and safety of Modified Spleen-Strengthening and Heart-Clearing Formula created by my tutor according to her experience were evaluated in the treatment of moderate or severe atopic dermatitis(AD),so was its effect on improving pruritus caused by the disease,in order to provide practical experience and guidance for the treatment of AD.Methods:The randomized controlled principle was followed in this study.Patients with moderate or severe AD conforming to the U.K.diagnostic criteria and aging from 2 to 35 were selected as the research objects.A total of 47 patients were enrolled and randomly divided into the treatment group and the control group.Their gender,age of onset,course of disease and whether the patients and their parents have other "atopic" history were recorded.Patients in the treatment group were given Modified Spleen-Strengthening and Heart-Clearing Decoction,Levocetirizine Hydrochloride Tablets(specification:5 mg),Compound Glycyrrhizin Tablets(specification:25 mg)for oral administration,while the control group was treated with Levocetirizine Hydrochloride Tablets and Compound Glycyrrhizin Tablets(same specifications with the above)for oral administration.Patients in the two groups all accepted external application of body moisturizer for at least twice a day(Children were given no less than 150 g a week,while adults 300 g).They were informed about the issues of diet,lifestyle and bath that needed to be paid attention to.4 weeks was a course of treatment,and continuous 3 courses of treatment were used.In the course of the study,the SCORAD score,pruritus score(VAS)and adverse reactions of patients before treatment,and the 2nd,4th,8th and 12th week after treatment were observed and recorded respectively to analyze the statistics of clinical efficacy so as to achieve conclusions.Results:1.Finally,40 patients met the requirements and completed the clinical study,20 cases in the treatment group(12 male,8 female),and 20 in the control group(10 male and 10 female).The gender,age,course of disease,and the scores of SCORAD and pruritus(VAS)for the first time of patients in the two groups were analyzed respectively.Results showed that there was no significant difference between the two groups(P>0.05),indicating that the general situation of patients in the two groups was comparable.2.After the treatment,the SCORAD scores of the two groups were analyzed.No significant difference was found after 2,4 and 8 weeks of treatment(P>0.05),but after 12 weeks,the two groups showed significant difference in the SCORAD scores(P<0.05).3.The pruritus scores(VAS)of patients in the two groups were compared.There was no significant difference after 2 and 4 weeks of treatment(P>0.05),but after 8 weeks,the changes of pruritus scores(VAS)in the two groups were statistically different(P<0.05).After 12 weeks of treatment,there was obvious difference in the changes of pruritus scores(VAS)between the two groups(P<0.01).4.The total effective rate was 85%in the treatment group,while 55%in the control group.In comparison of curative effects in the two groups,there was a statistically difference(P<0.05).No serious adverse reactions occurred in the whole process of treatment.Conclusions:From the view of pathogenesis of "spleen deficiency and heart fire",the rash and pruritus due to AD can be effectively improved by Modified Spleen-Strengthening and Heart-Clearing Formula with few side effects.The formula can be regarded as a safe and effective method in the treatment of AD.
