Background and objectiveAtopic dermatitis(AD) is a chronic inflammatory skin disease.The prevalence of AD has been increasing in recent years,and the 90%of cases appear in children and adolescent.The disease brought great impacts on the quality of life in AD patients and their parents.The contemporary western medicine for atopic dermatitis is just to ameliorate symptoms during the flare of the disease and such managements didn't display good therapeutic effects for those of refractory cases.It' s observed in the clinical practice that the Chinese medicine could improve quality of life of the patients through relief of the symptoms,enhancement of quality of sleep,regulation of the spleen and stomach,amelioration of anxiety and depression,and reinforcement of physical status.The entirety view of TCM is agreeable with the multi-dimension evaluation of the quality of life.It is essential for us to utilize the international received clinical evaluation instrument to make the objective estimate the chianese medicine in atopic dermatitis.We have translated the original version of the questionnaires of CDLQI and DFI into Chinese one and reevaluated the validity,reliability and responsiveness.Meanwhile,the impact of quality of life to atopic dermatitis patients and their parents were tested before and after treatment of the formula of Hilling and Clearing away Heart-fire.Methods1.Tbe translation of the queationaires:The programme of "the translations and back-translatiions"2.Questionaires survey method:there are 151 skin disease patients in group A,60 AD patients in group B,50 patients(not in skin disease) in group C,50 health children in group D.All children in the study are aged in 5-16 years.Ask the children in group A,B,C,D fill in the questionnaires of CDLQI.Ask the AD patients and their parents fill in the questionnaires of CDLQI and DFI separatedly,and fill in the two questionnaires again within 24 houres.After two weeks,bring back the questionnaires and fill in the two questionnaires thirdly in the next visit.3.Observation of clinical therapeutic effects:31 cases suffered from atopic dermatitis were admitted to this clinical tiral based on rigour admission criterion.Before treatment,1 month and 3 months after treatment of the formula,the evalution of quality of life by CDLQI and DFI and the values of SCORAD were collected respectively.What' s more,The general therapeutic effects was evaluated by the way of Nimodipine method 3 months after treatment.4.Statistics:The statistical significance of the outcomes were determined by t-test or Wilcoxon signed rank test through software package of SPSS 13.0.Results1.After receiving the formal permission,We create the translation into Chinese of the two questionnaires with the programme of "the translations and back-translatiions" Cronbach' sαcoefficient is 0.816,Guttman is 0.814 in the questionnaires of CDLQI;Cronbach' sαcoefficient is 0.926, Guttman is 0.893 in the questionnaires of DFI.2.The difference of quality of life between skin diseasepatients and AD patients is statistically significant(P＜0.001).The highest scoring questions are related to the effect of pruritus,sleep and recreation in CDLQI. The difference of quality of life between AD parents and common patients is statistically significant(P＜0.001).The highest scoring questions are related to the effect of tiredness,sleep and distress in DFI.3.31 cases suffered from atopic dermatitis were admitted to this clinical tiral based on rigour admission criterion.Before treatment,1 month and 3 months after treatment of the formula,the mean scoring of CDLQI is 15.67±7.51,11.33±5.59,7.22±4.602.The difference is statistically significant (P＜0.001).the mean scoring of DFI is 17.97±7.50,13.10±6.35,9.58±5.06. The difference is statistically significant(P＜0.001).Conclusions1.The simplified Chinese version of CDLQI and DFI is finished.The two questionnaires can be find on the web of http://www.dermatology.org.uk/. Through the psychometrics test,the evaluation of the validity,reliability and responsiveness is good.2.AD has an impact on the child and family' s quality of life.The much more significant problem that affect child is pruritus,sleep and recreation. The same problem that affect parents is tiredness,sleep and distress.3.The efficacy of the formula of hilling and clearing away heart-fire, demonstrated through the method of QoL evaluation,indicates that the Chinese medicine could improve the AD child and parents' s quality of life.
