| Background Ear sore is a disease characterized by diffuse swelling and pain in the external ear canal caused by the invasion of damp heat toxin into the ear orifice.It is equivalent to otitis external in western medicine.For ear sore,traditional Chinese medicine pays attention to "five diagnosis combined with reference",witch including look,listen,question,feel the pulse and check.The treatment varies according to the clinical symptoms and signs of patients.It is usually based on the external treatment method of traditional Chinese medicine.The treatment principle is to clear away heat,detoxify dampness and detumescence.Students have a certain understanding of the treatment of ear sore with external application of golden butter.The tutor thinks that this treatment method can make the effect reach the disease center directly and relieve the symptoms of ear sore.This method has the characteristics of simple operation,low cost and no obvious side effects.In this study,we take the patients with wind heat toxin as the research object,to observe the clinical efficacy and safety of Golden oil in the treatment of this type of ear sore.Objective To compare the therapeutic effect of Golden oil gauze and ofloxacin ear drops gauze in the treatment of wind heat and toxic ear sores,and to observe the relationship between the symptom and sign scores,the changes of VAS score of external auditory canal pain,influencing factors and clinical efficacy of the trial group and the control group before and after treatment.So as to evaluate the clinical efficacy and safety of Golden oil gauze in the treatment of ear sore caused by wind heat toxin.Methods From May 2019 to December 2019,70 otorhinolaryngology patients who met the inclusion criteria were randomly divided into the trial group and the control group.Each group had 35 patients.Before the treatment,he patients in the two groups were given external auditory canal cleaning(normal saline).In the trial group,the method of clearing away heat,detoxification,dampness and swelling was used,and the Golden oil gauze(drug composition: 500g of smallpox powder,250 g of rhubarb,phellodendron,turmeric,Angelica dahurica,100 g of Atractylodes macrocephala,Nanxing,liquorice,Magnolia officinalis and tangerine peel respectively)was applied externally.One tablet was taken once a day.In the control group,the gauze of ofloxacin ear drops was taken three times a day,six drops a time,and the course of treatment in both groups was 3 days.Collect the scores of each index in the course of treatment,and analyze and process the relevant data by medical statistics after the end of the experiment.Results 1 There were 35 cases in the trial group,including 9 cases of recovery,20 cases of significant effect,3 cases of effective effect,3 cases of ineffective effect,total effective 32 cases,total effective rate is 91.42%;there were 35 cases in the control group,including 0 case of recovery,11 cases of significant effect,15 cases of effective effect,9 cases of ineffective effect,total effective 26 cases,total effective rate is 74.28%.After statistical analysis,the difference between them was statistically significant(P < 0.01).2 After treatment,the VAS score of the trial group was lower than that of the control group,and the difference was statistically significant(P < 0.01).3 After treatment,the score of symptoms and signs of otodynia,hyperemia,swelling of ear canal wall,ear itching and ear burning in the trial group were lower than those in the control group.The difference between them was statistically significant(P < 0.01).The score of symptoms and signs of headache,fever and dry stool in the trial group was compared with that in the control group.There was no significant difference between the two groups(P > 0.05).4 Observe the relationship between sex and curative effect between the trial group and the control group.trial group: In male,total effective 10 cases,total effective rate is 90.91%;Among famale,total effective 22 cases,total effective rate is 91.67%,after statistical analysis,there was no significant difference between them(P > 0.05).control group: In male,total effective 17 cases,total effective rate is 80.95%;Among famale,total effective 9 cases,total effective rate is 64.29%,after statistical analysis,there was no significant difference between them(P > 0.05).5 Observe the relationship between the age and the curative effect of the trial group.Patients aged 12-30 years old,10 cases were effective,total effective rate is 100%;patients aged 31-50 years old,17 cases were effective,total effective rate is 100%;patients aged 51-65 years old,5 cases were effective,total effective rate is 62.5%.After statistical analysis,there was no significant difference among the three groups(P > 0.05)6 Observe the relationship between the influence factors and the curative effect of the trial group.The relationship between ear digging habits and the curative effect of the trial group.After statistical analysis,the difference was statistically significant(P < 0.05).There was no significant difference between the aspects of diet preference,smoking,drinking,staying up late,external auditory canal water inflow and the curative effect of the trial group(P > 0.05).7 There were no adverse reactions in the treatment of the trial group and 1 case of aggravation of the pain symptoms in the control group.Conclusion 1 External application of Golden oil in the treatment of ear sore has significance effection,which was better than that of the control group.2 External application of Golden oil can effectively improve the symptoms of otodynia,and the degree of pain improvement is significantly better than the control group.3 External application of Golden oil can improve the symptoms and signs of the trial group,witch include otodynia,hyperemia,swelling of ear canal wall,ear itching and ear burning.the effection was significantly better than that of the control group.4 External application of Golden oil in the treatment of ear sore.there was no significant difference between the gender,age,preference of diet,smoking,drinking,staying up late,the water in the external auditory canal and the curative effect of the trial group.5 The therapeutic effect of the external application of Golden oil is related to the habit of ear digging.The more the patients are accustomed to ear digging,the slower the therapeutic effect.6 External application of Golden oil in the treatment of ear sore is no obvious adverse reaction,so it is worth popularizing. |
