Clinical Study On The Changes Of Inflammatory Reaction In Contralateral Eyes After Phacoemulsification Combined With Intraocular Lens Implantation

Objective: This dissertation is to assess what changes will take place at different times in concentrations of inflammatory factor IL-4,IL-10,IL-17 A,and IFN-? in the simple senile cataract patients' aqueous humor of contralateral eye after the sequential phacoemulsification and intraocular lens implantation on first-eye,so as to figure out whether the cataract surgery on first-eye will have an impact on the inflammatory state of the second eye and how the impact will change after surgery over time.Methods: 36 patients with bilateral simple senile cataract who met the inclusion criteria of this experiment between May 2018 and November 2019 were randomly divided into three groups: Group A(operation interval ? 5 days and ? 10 days),in which the first operation eye was A1 group and the second operation eye was A2 group;group B(operation interval ? 11 days and ? 28 days),in which the first operation eye was B1 group and the second operation eye was B2 group;group C(operation interval > 28 days),in which the first operation eye was C1 group and the second operation eye was C2 group.Firstly,all patients were regularly treated with levofloxacin eye drops(6 times a day)three days before the operation.Also,all groups were treated with phacoemulsification and intraocular lens implantation by the same experienced surgeon,with aqueous humor collected before the operation.Furthermore,while the concentrations of IL-4,-10,-17 a and IFN-? in aqueous humor were measured by electrochemical luminescence detection(MSD),the aqueous humor cells,aqueous flash and best corrected visual acuity in 1,3 and 14 days after the operation were observed and compared with those before the operation.Results: All 36 patients were followed up,and there were no complications occurred during and after the operation.1)General situation: there were no significant difference in gender and age between groups A,B and C(P > 0.05);there were no significant difference in nuclear grade,degree of intraocular lens,ultrasound energy and effective ultrasound time between groups A1,A2,B1,B2,C1 and C2(P > 0.05);2)Inflammatory factors:(1)The concentrations of IFN-? in group A1 were lower than that in group A2,the concentrations of IL-10,IL-17 A and IL-4 in group A1 were higher than those in group A2,and the difference was not statistically significant(P > 0.05);the concentrations of IFN-?,IL-10,IL-17 A and IL-4 in group B1 were lower than those in group B2,and the difference was not statistically significant(P > 0.05);the concentrations of IFN-?,IL-10,IL-17 A and IL-4 in group C1 were lower than those in group C2,and the difference was not statistically significant(P > 0.05);(2)There was no statistical difference in the concentrations of cytokines in aqueous humor between groups A1,B1 and C1(P > 0.05);(3)There was no statistical difference in the concentrations of cytokines in aqueous humor between groups A2,B2 and C2(P > 0.05);(4)There was no statistical difference in the concentrations of cytokines in aqueous humor's D-value of the first-eye and second-eye between groups A,B,C(P > 0.05);3)Aqueous flash:(1)The aqueous flash of the eyes in each group gradually decreased with the prolongation of time,and disappeared at 14 days after operation;(2)There was no statistical difference between groups A1 and A2 at the same time point(P > 0.05);there was no statistical difference between groups B1 and B2 at the same time point(P > 0.05);there was no statistical difference between groups C1 and C2 at the same time point(P > 0.05);(3)There was no statistical difference between groups A1,B1 and C1 at the same time point after operation(P > 0.05);(4)There was no statistical difference between groups A2,B2 and C2 at the same time point after operation(P > 0.05);4)Aqueous humor cells:(1)The aqueous humor cells of the eyes in each group gradually decreased with the prolongation of time,and almost disappeared at 14 days after the operation.(2)There was no statistical difference between groups A1 and A2 at the same time point(P > 0.05);there was no statistical difference between groups B1 and B2 at the same time point(P > 0.05);there was no statistical difference between groups C1 and C2 at the same time point(P > 0.05);(3)There was no statistical difference between groups A1,B1 and C1 at the same time point after operation(P > 0.05);(4)There was no statistical difference between groups A2,B2 and C2 at the same time point after operation(P > 0.05);5)Visual acuity comparison:(1)The visual acuity of both eyes in each group was improved before operation;(2)There was no statistical difference between groups A1 and A2 at the same time point(P > 0.05);there was no statistical difference between groups B1 and B2 at the same time point(P > 0.05);there was no statistical difference between groups C1 and C2 at the same time point(P > 0.05);(3)there was no statistical difference between groups A1,B1 and C1 at the same time point before operation,1 day,3 days and 14 days after the operation(P > 0.05);(4)there was no statistical difference between groups A2,B2 and C2 at the same time point before the operation,1 day,3 days and 14 days after the operation(P > 0.05).Conclusion: For simple senile cataract patients,successful cataract phacoemulsification in first-eye will not cause significant changes in the inflammation of the second-eye,suggesting that the length of the interval between cataract operations on those patients has no impact on the surgical effect.
