Analysis Of Factors Affecting Doctor-patient Conflict In Pediatrics Based On The Scale Of Doctor-patient Conflict Factors

Objective:To develop a pediatric doctor-patient conflict measurement scale suitable for local use in China.This scale can provide a tool for medical staff to quantify the conflict between medical staff and patients and their relatives,so as to facilitate clinical staff in hospitals to identify and measure the conflict in a timely manner,so as to improve staff's ability to pre-process the conflict.It also provides the basis for timely intervention,so as to reduce or avoid the occurrence of conflict,and also provides some reference basis for the study of doctor-patient conflict in pediatrics.Methods:This study used semi-structured interview and literature review to analyze and summarize the causes of doctor-patient conflict,and initially formed the"evaluation index of doctor-patient conflict in pediatrics".Subsequently,two rounds of Delphi expert letter consultation method were adopted to modify the relevant indicators.Finally,through the questionnaire survey of medical personnel,the formal scale was determined and the clinical application was carried out to judge the main factors affecting the doctor-patient conflict in pediatrics.Results:In the revision of the pediatric doctor-patient conflict scale by Delphi method,the positive coefficient of the two rounds of experts was 80%and 100%,respectively,and the authority coefficient of the two rounds of experts was 0.938.The expert coordination degree of the scale was highly consistent,and the coefficient of variation of all indicators was<0.25.In the reliability test,the cronbach coefficient of homogenous reliability of the scale is 0.886,indicating that the scale has a good reliability.In the validity test,the content validity of the scale was 0.92.After exploratory factor analysis,it was found that the load on the factors corresponding to each item was above 0.76.In the confirmatory factor analysis,it was concluded that?~2/df<3,RMSEA<0.08,NFI,GFI and CFI values were all above 0.9,indicating that the scale had good content validity and structure validity.The application and analysis results of the scale show that the title,the number of conflicts and the department of the medical staff are the main factors affecting the conflicts between doctors and patients in pediatrics.Conclusion:the utility and reliability of the pediatric doctor-patient conflict scale are relatively good,which can objectively and accurately reflect the problems related to doctor-patient conflict in the pediatric work of the hospital to a certain extent.Therefore,the paediatric doctor-patient conflict measurement scale has certain reference value in solving the problem of paediatric doctor-patient conflict.
