Objective: Based on Voxel-based morphometry technology and pain behavioral technology,observe the correlation and characteristics of brain structure and morphological changes in LDH chronic lower limb pain model rats,and reveal the brain mechanism of LDH chronic lower limb pain.To investigate the effects of DuzhongYaotong Pills on the brain structure and morphology of LDH chronic lower limb pain model rats,and to clarify the action effect of Duzhong Yaotong Pills and to provide theoretical and experimental basis for its better clinical application.Methods:(1)VBM study of LDH chronic lower limb pain model rats: Twenty-four SD male rats,6-8 weeks old,with a body weight of(250±20)g,were selected as the research objects and randomly divided into three groups: Normal group,Sham-LDH group and LDH group,A model of autologous nucleus pulposus transplanted rats with chronic lower limb pain was established in the LDH group.The pain behavioral observation was performed in each group at 0d before modeling and at 2d,7d,14 d,21d and 28 d after modeling,mainly including general behavioral observation,mechanical pain threshold measurement,and thermal pain threshold measurement.Four rats in each group were randomly selected to collect T2 weighted structural images and VBM images at 0d before modeling and at 14 d and 28 d after modeling,Statistical methods were used for the analysis of the data.(2)Study on the VBM of Duzhong Yaotong Pills in the treatment of LDH chronic lower limb pain: Seventy-two SD male rats were selected,aged 6-8 weeks,with a body weight of(250±20)g.They were randomly divided into three groups: Normal group,LDH group and Treatment group,A model of autologous nucleus pulposus transplanted rats with chronic lower limb pain was established in the LDH group and Treatment group.The Treatment group was given Duzhong Yaotong Pills medicine at a dose of 259.2mg/Kg twice a day,and the Normal group and the LDH group were given the same dose of saline.The pain behavioral observation was performed in each group at 0d before treatment and at 2d,7d,14 d,21d and 28 d after treatment,mainly including general behavioral observation,mechanical pain threshold measurement,and thermal pain threshold measurement.Four rats in each group were randomly selected to collect T2 weighted structural images and VBM images at 0d before treatment and at 14 d and 28 d after treatment,Statistical methods were used for the analysis of the data.Results:(1)There was no significant difference in MWT value between the three groups of rats before modeling(P>0.05);There significantly from 2d to 14 d after modeling,and the MWT value was stable from 14 d to 28 d after modeling;Compared with rats in Normal group and Sham-LDH group,the MWT value in LDH group were statistically significant at 2d,7d,14 d,21d,and 28 d after modeling(P<0.05).(2)There was no significant difference in TWL value between the three groups of rats before modeling(P>0.05);There was no significant difference in TWL value between Sham-LDH group and Normal group at the 2d,7d,14 d,21d,and 28 d after modeling(P>0.05);In the LDH group,the TWL value of the rats decreased significantly from 2d to 14 d after modeling,and the MWT value was stable from 14 d to 28 d after modeling;Compared with rats in Normal group and Sham-LDH group,the TWL value in LDH group were statistically significant at 2d,7d,14 d,21d,and 28 d after modeling(P<0.05).(3)In the part of VBM study of LDH chronic lower limb pain model rats,There were interactions between the brain structure regions of the three groups of rats at different time points,and the differences were statistically significant(P<0.05).Gray matter volume was reduced in these brain regions,Specifically,Hypothalamic Region L、Subiculum L、Secondary Motor Cortex R、Striatum R、Septal Region R、Cornu Ammonis R、Corpus Callosum and Associated Subcortical Gray Matter L and R、Molecular Layer of the Cerebellum L and R.(4)The MWT value of rats in Normal group and LDH group remained relatively stable from 0d before treatment to 28 d after treatment;There was no significant difference in the MWT value of rats in the Treatment group between 0d before treatment,2d and 7d after treatment compared with the MWT value of LDH group at the same time(P>0.05);The MWT value of rats in the Treatment group were compared with the LDH group at the same time at 14 d,21d,and 28 d after treatment,and the differences were statistically significant(P<0.05);The MWT value of rats in the Treatment group were compared with those of the Normal group at the same time at 21 d and 28 d after treatment,and there was no significant difference(P>0.05).(5)The TWL value of rats in Normal group and LDH group remained relatively stable from 0d before treatment to 28 d after treatment;There was no significant difference in the TWL value of rats in the Treatment group between 0d before treatment,2d and 7d after treatment compared with the TWL value of LDH group at the same time(P>0.05);The TWL value of rats in the Treatment group were compared with the LDH group at the same time at 14 d,21d,and 28 d after treatment,and the differences were statistically significant(P<0.05);The TWL value of rats in the Treatment group were compared with those of the Normal group at the same time at 21 d and 28 d after treatment,and there was no significant difference(P>0.05).(6)In the part of Study on the VBM of Duzhong Yaotong Pills in the treatment of LDH chronic lower limb pain,There were interactions between the brain structure regions of the three groups of rats at different time points,and the differences were statistically significant(P<0.05).Gray matter volume was increased in these brain regions,Specifically,Lateral Entorhinal Cortex Internal part L、Primary Somatosensory Cortex L、Olfactory Bulb R、Ectorhinal Cortex R、Retosplenial Dysgranular Cortex R、Cornu Ammonis R、Corpus Callosum and Associated Subcortical Gray Matter L and R.Conclusion:(1)A model of autologous nucleus pulposus transplanted rats with chronic lower limb pain was successfully established,and a long-term reduction of the MWT value and TWL value emerged and the hyperalgesia occurred after the modeling.(2)Morphological changes in the brain structure of LDH chronic lower limb pain model rats can be found with VBM technology.(3)Duzhongyaotong Pills can increase the MWT value and TWL value of LDH chronic lower limb pain model rats,It can relieve the chronic pain of LDH.(4)Duzhong Yaotong Pills can partially change or reverse abnormal brain morphology and structure,and exert analgesic effect by treating "brain".
