Objective: Based on resting functional magnetic resonance imaging and pain behavior,the changes of brain structure and morphology in rats with LDH chronic lower limb pain were studied,and the effects of Duzhong Yaotong Pill on brain structure and morphological changes in LDH rats with chronic lower limb pain were discussed,and clarify Eucommia low back pain The mechanism of action of the pill provides a theoretical basis for the better clinical application of Duzhong Yaotong Pll.Methods:(1)RS-fMRI study of LDH chronic lower limb pain model rats: 24 healthy SD male rats aged 6-8 weeks,weighing 250±20g,numbered according to body weight,and randomly divided into 3 groups: normal group,sham operation group,model Group,and model group used autologous nucleus pulposus transplantation to establish LDH chronic lower limb pain rat model.Each group underwent gross behavioral observation,mechanical pain threshold measurement,and thermal pain threshold measurement on 0 days before modeling,2 days,7days,14 days,21 days,and 28 days after modeling.Four rats in each group were randomly selected to collect BOLD data using Bruker’s 7.0T nuclear magnetic field at 0 days before modeling,14 days after modeling,and 28 days after modeling.The BOLD data were processed by methods such as Regional Homogeneity(ReHo)and Fractional Amplitude of Low Frequency Fluctuation(f ALFF),and statistical analysis was performed on the obtained data.(2)The RS-fMRI study of Duzhong Yaotong Pills in the intervention of LDH chronic lower limb pain: 72 healthy SD male rats aged 6-8 weeks,weighing 250±20g,numbered according to body weight,and randomly divided into 3 groups:normal Group,model group,treatment group,model group and treatment group adopt autologous nucleus pulposus transplantation to establish LDH chronic lower limb pain rat model.The treatment group was given Duzhongyaotong pill intragastrically,the dose was 10 ml kg-1,twice a day,the normal group and the model group were given the same amount of normal saline by intragastric administration.Each group underwent gross behavioral observation,mechanical pain threshold measurement,and thermal pain threshold measurement on 0 days before administration,2 days,7 days,14 days,21 days,and 28 days after administration.Four rats in each group were randomly selected 0 days before modeling,14 days after modeling,and 28 days after modeling.Bruker’s 7.0T nuclear magnetic field was used to collect BOLD data,and ReHo,f ALFF and other methods were used to process the BOLD data.Perform statistical analysis.Results:(1)There was no statistically significant difference in PWT between the three groups of rats before modeling(P>0.05);there was no significant difference between the sham-operated group and the normal group at each time point after modeling(P>0.05),but there was significant difference between the model group and the normal group and the sham-operated group at each time point after modeling(P<0.05).(2)There was no statistically significant difference in TWL between the three groups of rats before modeling(P>0.05);there was no significant difference in TWL values between the sham operation group and the normal group at each time point after modeling(P>0.05);Compared with the normal group and the sham operation group,the TWL values of the rats in the group were statistically significant(P<0.05).(3)In the RS-fMRI study part of LDH chronic lower limb pain model rats,the three groups of rats had an interactive effect on the brain structure at different time points,and the difference was statistically significant(P<0.05).The brain map results show that the brain regions with significant differences in ReHo values are: Periventricular Grey L and Striatum R;brain regions with significant differences in f ALFF values are: Striatum R,Olfactory Bulb L,and bilateral Molecular Layer of the Cerebellum.(4)The PWT values on the 21 st and 28 th days after the administration of the rats in the treatment group were compared with 0 days before administration,2 days,7 days and 14 days after administration,the difference was statistically significant(P<0.05);The PWT values of rats in the treatment group were compared with those in the model group at the same time point at 14 days,21 days,and 28 days after administration,and the difference was statistically significant(P<0.05);There was no significant difference in PWT between the treatment group and the normal group at the same time point 21 and 28 days after administration(P>0.05).(5)At 21 and 28 days after administration,the TWL value of rats in the treatment group was significantly higher than that at 0 days before administration,2 days,7 days and 14 days after administration(P<0.05);The TWL values of rats in the treatment group were compared with those in the model group at the same time point at 14 days,21 days,and 28 days after administration,and the difference was statistically significant(P<0.05);There was no significant difference in TWL between the treatment group and the normal group at the same time point 21 and 28 days after administration(P>0.05).(6)In the RS-fMRI study part of LDH chronic lower limb pain model rats,the three groups of rats had an interactive effect on the brain structure at different time points,and the difference was statistically significant(P<0.05).The results of encephalogram showed that the brain regions with significant increase of Reho value were : Primary Somatosensory Cortex L and f ALFF: Entorhinal Cortex L and Cornu Ammonis R.Conclusion:(1)The mechanical pain threshold and thermal pain threshold were significantly decreased after the LDH model was established by autologous nucleus pulposus transplantation in rats,and the hyperalgesia occurred,indicating that the model of LDH rats with chronic lower limb pain was successful.(2)Using RS-fMRI technology,from ReHo and fALFF analysis indicators,it can be found that the structure of the relevant brain area in the LDH chronic lower limb pain model rat has undergone morphological changes.(3)Duzhong Yaotong Pill can increase the mechanical pain threshold and thermal pain threshold of LDH chronic lower limb pain model rats,and has the effect of relieving LDH chronic pain.(4)Duzhong Yaotong Pill can partially improve or reverse the abnormal brain shape and brain structure,and play an analgesic effect by treating the "brain".
