Objective:To observe the clinical efficacy of Duzhong Yaotong Pill in the treatment of residual symptoms of lumbar disc herniation after percutaneous spinal endoscopy, and to explore its mechanism of action. It provides a theoretical basis for the treatment of residual symptoms of percutaneous spinal endoscopy. Methods:During the period from August 2017 to August 2018,60 patients with lumbar disc herniation who had residual symptoms after percutaneous spinal endoscopic surgery were treated in the minimally invasive orthopedics of the spine of Gansu Provincial Hospital of Traditional Chinese Medicine. All patients were single-segment lumbar disc herniation.According to the random number table method, randomized grouping was performed, and the control group was given routine treatment after operation. The treatment group was given 8capsules of Eucommia lupus puncture pills in the hospital on the basis of routine postoperative treatment,3 times a day.At the same time guide the two groups of patients with functional exercise. The follow-up period was 6 months.Follow-up was performed at 1month,3 months, and 6 months after surgery. The corresponding rating scale was filled out.We used Visual Analogue Scale(VAS)to evaluate patients before and after surgery. At the various stages of low back pain, the Japanese Orthopedic Association Scores(JOA)evaluated the improvement of postoperative lumbar spine function, and the Oswestry Disability Index(ODI)assessed the quality of life of patients.Clinical efficacy of the two groups. The clinical efficacy of the patients was assessed by modified Macnab efficacy evaluation criteria 6months after surgery.Result:1.A total of 60 patients were enrolled, including 30 patients in the treatment group,17males and 13 females, with an average age of(53.03±6.38)years old,and the proportion of patients with prominent segments located at L3/4,L4/5,and L5/S1 were 3 cases,14 cases,13 cases, the average BMI index was(25.46±1.77)Kg/m~2;30 cases in the control group,18males and 12 females, the average age was(53.53±6.36)years old,in which the prominent segment was 2 patients,16 patients, and 12 patients with L3/4,L4/5,and L5/S1.The average BMI index was(24.49±1.97)Kg/m~2.There were no significant differences in gender, age, prominent segment, and BMI index between the two groups(P>0.05),which were comparable.2.VAS scores:The VAS scores showed a downward trend in the two groups. The VAS scores in the control group decreased from March to June(P>0.05),and the other follow-up time points were compared. At the same time, the VAS scores were significantly decreased(P<0.05).Comparison between the two groups:At 1 month after operation, the VAS scores of the two groups were basically the same as those before the operation, and the difference was not statistically significant(P>0.05).At 3 months and 6 months after the operation, the treatment group VAS The decline of the score was significantly greater than that of the control group, and the difference was statistically significant(P<0.05).3.JOA score results:Compared with the two groups, the JOA scores showed an upward trend, and the increase rate was significant in the two groups(P<0.05).Comparison between the two groups:At 1 month after operation, the JOA scores of the two groups were basically the same as those before the operation, and the difference was not statistically significant(P>0.05).The JOA score of the treatment group was 3 months and 6 months after operation. The increase was significantly greater than that of the control group, and the difference was statistically significant(P<0.05).4.ODI scores:ODI scores showed a downward trend when compared between the two groups, and the decrease was significant when the two groups were compared(P<0.05).Comparison between the two groups:At 1 month after operation, the ODI scores of the two groups were basically the same as those before the operation, and the difference was not statistically significant(P>0.05).The ODI score of the treatment group was 3 months and 6months after operation. The decrease was significantly greater than that of the control group, and the difference was statistically significant(P<0.05).5.At the 6-month follow-up,the total excellent and good rates were 93.3%and 83.3%,respectively.There was no significant difference in the total excellent rate between the two groups at the two follow-up time points(P>0.05).During the 6-month follow-up period,1patient in the control group developed postoperative recurrence and underwent secondary PELD surgery. Conclusion:Duzhong Yaotong Pill can effectively improve postoperative residual symptoms in patients with lumbar disc herniation, which is beneficial to the recovery of postoperative lumbar function and improve the quality of postoperative life.
