Objective:This study was to understand the status quo of gynecological cancer chemotherapy patients with anxiety,depression,and to analyze the data of general,sleep,fatigue effects on anxiety,depression,and explore the self-service mindfulness intervene in the anxiety and depression in patients with the clinical effect,to reduce patients' anxiety,depression provides a simple,safe and effective,cost saving method,the dual economic and social benefit.Methods:Part ?: The current situation survey.A questionnaire was randomly selected from200 patients with gynecologic cancer chemotherapy who met the inclusion criteria,including general data Questionnaire,anxiety self-rating scale,depression self-rating scale,Pittsburgh Sleep Quality Index and cancer-induced fatigue scale.Part II: Clinical intervention.Patients with depression score ?53 and/or anxiety score ?50 were selected from the current situation survey as study subjects.The patients were randomly divided into control group and intervention group,the control group was given routine care,and the intervention group was given self-help mindfulness intervention on the basis of routine care.Results:The first part is to investigate the current situation of anxiety and depression in gynecological cancer patients undergoing chemotherapy.1.The anxiety score was(48.89±6.57),and the detection rate was 49.5%.The score of depression was(58.70±9.94),and the detection rate was 74.5%.Among them,99 were anxious,149 were depressed,and 77 were anxious and depressed at the same time.2.The scores of anxiety and depression in patients aged 18-49,those with no employment,primary school education or below,per capita family income <2000yuan,side effects?4,and those in rural remote areas were significantly higher than those in other groups(P<0.01).Sleep and fatigue were the important influencingfactors of anxiety and depression,which were positively correlated(r=0.276,P<0.01;r=0.173,P<0.05),that is,the poorer sleep quality,the more serious fatigue,the more serious anxiety and depression in patients.The second part is the effect of self-help mindfulness intervention on anxiety anddepression in gynecological cancer patients undergoing chemotherapy1.Comparison of baseline data including general demographic data,anxiety,depression,sleep and fatigue between the two groups before intervention showed no statistically significant difference(P>0.05).2.After the intervention2.1 intra-group comparison(1)the difference of anxiety scores before and after intervention in the control group was statistically significant(P<0.05).The anxiety scores of the intervention group before and after intervention were(48.89 ± 5.15)and(43.17 ± 4.40),respectively,with statistically significant differences(P<0.01).(2)there was a statistically significant difference in depression scores in the control group before and after intervention(P<0.01).Before and after intervention,depression scores in the intervention group were(63.45±5.27)and(56.04±3.91),respectively,with statistically significant differences(P<0.01).(3)the difference of sleep scores before and after intervention in the control group was statistically significant(P<0.01).The sleep scores of the intervention group before and after intervention were(11.23±3.07)and(8.38±2.14),respectively,with statistically significant differences(P<0.01).(4)there was a statistically significant difference in the scores of patients with cancer-related fatigue before and after intervention in the control group(P<0.05).The scores of cancer-related fatigue before and after intervention in the intervention group were(26.72 ± 5.06)and(23.81 ± 2.10),respectively,with statistically significant differences(P<0.01).2.2 comparison between groups(1)The anxiety scores of the two groups were(43.17±4.40)points and(47.79±4.90)points,respectively,with statistically significant differences(P<0.01).(2)The depression scores of the two groups were(56.04 ± 3.91)points and(58.64±4.22)points,respectively,with statistically significant difference(P<0.01).(3)The sleep scores of the two groups were(8.38±2.14)points and(9.57±2.68)points,respectively,with statistically significant differences(P<0.05).(4)The scores of cancer-related fatigue in the two groups were(23.81±2.10)points and(25.57±2.58)points,respectively,with statistically significant difference(P<0.01).Conclusion:1.The incidence rates of anxiety and depression in gynecological cancer patients undergoing chemotherapy were 49.5% and 74.5%,respectively,which were at a high level.They were affected by age,working status,education,residence,monthly family income,side effects,sleep and fatigue,and were positively correlated with sleep and fatigue.2.Self-help mindfulness can effectively relieve anxiety and depression in gynecological cancer patients undergoing chemotherapy,improve sleep quality and alleviate fatigue symptoms,which is worthy of clinical promotion and application.
