Objective To analyze the QOL and influencing factors of elderly patients with chronic diseases,and measure the needs of daily care services in long-term care.To help policy makers allocate health resources rationally,and to provide a reference for meeting the needs of elderly with chronic diseases effectively.Method The data from the 2015 China Health and Retirement Longitudinal Study(CHARLS)was used to summarize and sort out the data.Descriptive analysis was performed by using spss20.0,and chi-square test was used to analyze the influencing factors of five dimensions of EQ-5D in elderly patients with chronic diseases.We constructed Multiple linear regression regression models to analyze the relationship between the QOL of elderly with chronic diseases and the influencing factors.The long-term care needs of the daily care were analyzed by using disability,quality-adjusted life years and long-term care service pricing standards.Results There is a difference in the QOL between the elderly with chronic and the general population.The QOL of the elderly with chronic is lower than that of the general population.Among the major chronic diseases,elderly patients with hypertension have the highest quality of life and the elderly patients with cancer have the lowest quality of life.The five-dimensional health level of elderly patients with chronic diseases is lower than that of the general population,and the most serious dimension are “pain/discomfort” and “anxiety/depression”.The EQ-5D five-dimensional health impact of chronic diseases on women's is greater than that of men,including “action”,“self-care”,“daily activities”,and “pain/discomfort”;The reducing of quality of life of oldest-old caused by chronic diseases is lower than that of the young aged;The EQ-5D five-dimensional health impact of chronic diseases of smokers is greater than that of non-smokers,including “action”,“pain/discomfort”,and “anxiety/depression”;The impact of chronic diseases on the quality of life of drinkers is greater than that of non-drinkers in“daily activities”,but in the two dimensions of “pain/discomfort” and “anxiety/depression”,the health effects of chronic diseases on non-drinking are greater than those of drinkers.In the “daily activities”,“pain/discomfort” and “anxiety/depression” dimensions,the quality of life impact of chronic diseases on those with insufficient sleep is greater than those with adequate sleep.Multiple linear regression results can be found that the quality of life value of elderly patients with chronic diseases is related to age,education level,region,smoking,drinking,depression,and sleep.The remaining QALYS for the general population was 21.51 years,and the remaining QALYS for the elderly with chronic was 19.37 years.The per capita QALYS loss of elderly people over 60 years old caused by chronic diseases was 1.81 years.In 2015,the total cost of long-term care about daily care services for 4435 elderly with chronic diseases was about 2,405,850 yuan.Under the assumption that the long-term care needs of the elderly remain unchanged,combined with the remaining QALYS of each elderly,the cost of long-term care about daily care services in future survival time is at least about 4,638,587,7.5 yuan.Conclusion The QOL of elderly with chronic diseases is lower than that of the general population,and chronic diseases have led to reducing in the quality of life in elderly.Improving the quality of life of the elderly can focus on the elderly,young people,and the healthy behaviors of the elderly such as smoking,drinking,and sleep.The elderly with chronic diseases have more mild disability,and long-term care needs are greater.
