Objective:To understand the background,necessity,connotation and types of the"medical and old-age care fusion" in the current domestic and foreign.Analysis the problems and optimization measures of the development,current pension model in China.Through the integrated,optimization of medical and pension resources,it establishes a comprehensive service platform for collaborative innovation.Explore and perfect the service mode of "medical and old-age care fusion" and its effective long-term care method.Provide reference for the study of public health services,and provide reference for the relevant departments to make decisions.Methods:A comparative study was adopted before and after the use of control.The research of 70 elderly patients with chronic diseases in the hospital was studied.The implementation of the integration of medical care,to provide the treatment and rehabilitation of the disease,health care,daily life care and other services.The evaluation index collection and check in after 6 months,including the self-care ability of the Barthel index,the recurrence of the disease,the treatment and nursing cost,and A variety of catheter infections.Then,Statistical related data,testing the effectiveness of the measures,analysis of effect of "medical and old-age care fusion" for elderly people,come to a conclusion.Descriptive statistical analysis was carried out using SAS 9.4 software package.Results:1.The study group included 70 people,male 44,female 26,mean age 78.6 years.The study of gender,age,disease type and number,type of health insurance data,in the pre intervention and at 6 months after the intervention had no significant difference,comparable.2.After 6 months,the self-care ability(ADL)Barthel index rose from 70 points to 80 points.It has improved,the difference was statistically significant(P<0.001).3.After 6 months,the recurrence of the disease lead to referral hospital,before the number of transferred,by an average of 2.9 times,reduced to 0.24 times.The difference was statistically significant(P<0.001).The recurrence of the disease.4.After 6 months,the cost of medical care expenses,from the previous year average of 94300 yuan,down to 49200 yuan.The cost of medical expenses reduced.The difference was statistically significant(P<0.001).5.A total of 15 patients with long-term indwelling catheter,the number of 1 to 3 kinds of catheter,only 1 cases of urinary tract infection due to urinary catheter.Conclusion:The care model of 'medical and old-age care fusion' is combined with the basic life care and health care services.It shows the model of 'medical and old-age care fusion' can improve the self-care ability,the quality of life,reduce the times disease recurrence,delay the complications and reduce the disability rate.It has a good effect to save the cost of social and family.In the face of the current situation of China's aging,the care model is an indispensable part of the pension,health insurance system.Imperative,it is worth promoting.
