Quality Of Life Investigation Of Disabled Elderly And Study On Their Demand And Utilization Of Community Resource

Purpose Evaluate the health status,quality of life and the need and utilization of community resources refer to the disabled elderly; Analyze the main influence factors of quality of life and the need and utilization of community resources among the disabled elderly people;Give suggestions for improving the quality of life and establishing the long-term care system in community providing for the disabled elderly.Method Questionnaire research were used among 950 disabled elderly people over 60-years-old who lived in two community in haidian district in beijing from May 2013 to December. The contents involved general population information,Activities of Daily Living Scale(ADL), Self-Rating Anxiety Scale (SAS), The Geriatric Depression Scale (GDS-15),Memorial University of New foundland Scale of Happines(MUNSH), Social Science Research Solutions(SSRS), the MOS 36 item short form health survey (SF-36), the survey of the need and utilization of community resources. Epidata 3.1 was used for data entry and SPSS 18.0 was adopted to perform frequency and descriptive analysis,comparative analysis, single factor analysis,x2 test and multivariate linear stepwise regression analysis. All the level of significance is 0.05.Result 1、In the formal survey, we received 944 questionnaires, the valid questionnaires were 902, and the effective rate reached 96.0%.40.5% disabled elderly were male,females accounted for 59.5%.The average age was 80.31±7.18 years old.52.4% of these people educated in middle schools and even lower.49.3% disabled elderly bereft their spouse.42.9% of their monthly income was less than RMB 2000 Yuan.2 The rate of having chronic diseases among these people was 97.6%. Anxiety and depression symptoms prevalence was 14.9% and 27.4% respectively.40.5% disabled elderly had a lower happiness degree.The main score of the social support was 28.81±6.36,lower than the Chinese norm 3、 The total score of QOL of disabled elderly was 101.85±16.59. Furthermore, the result of grade demonstrated good for 19.9%. medium for 75.7%, poor for 4.4%.Compared with other city in China,our data showed that the disabled elderly people in the research had lower quality of life. Single factor analysis of variance showed that their sex,age, monthly income, main care provider, care time of everyday,disabled severity degree, ADL, anxiety, depression, happiness and social support showed significant difference in total life quality of the disabled elderly people;Multivariate linear stepwise regression analysis indicated that action, care time of everyday, depression, happiness and anxiety were importment factors affecting the quality of life.、75.7% elders preferred to be cared in home.The top fourth demand of community health services of the disabled elderly were periodical physical examination (85.7%) emergency rescue (81.2%) establish of health files (78.7%) and home health care provided by medical personnel (70.3%);The top fourth utilization of community resources were housemaid/hourly employee (26.0%),family doctor (25.7%) and recreation center for aged (22.0%),home service (10.8%)Conclusion The results showed that the disabled elderly’s health status and life quality were worsened than normal and long-term care demand and utilizatoin were imbalance. Family, community health agencies and social forces should pay more attention on these people. According to the actual conditions of their QOL, care situation and the influencing factors, targeted measures should be worked out to develop and improve community long-term care system for disabled elderly, in order to comprehensively improve the QOL of them.
