Objective To investigate the significance of serum carbohydrate antigen 125(CA125)level in the diagnosis of Pulmonary hypertension(pulmonary hypertension,PH)in patients with Chronic Obstructive Pulmonary Disease(COPD).Methods A total of 130 patients with stable COPD in the department of geriatrics,respiratory inpatient departments of affiliated hospital of qinghai university from November 2018 to January 2020 were selected as the study subjects.Pulmonary Arterial Systolic Pressure(PASP)was calculated by the tricuspid regurgitant velocity(TRV)detected by echocardiography..The included cases were divided into the control group(COPD group only)and the observation group(COPD group with PH).The serum CA125 level of the two groups was compared to investigate the significance of serum CA125 level in the diagnosis of COPD patients with pulmonary hypertension.At the same time,the observation group was divided into mild group,moderate group and severe group according to the level of pulmonary artery systolic blood pressure of the patients,to explore the significance of serum CA125 level on the severity of pulmonary artery hypertension in COPD patients.SPSS25.0 was used for statistical analysis of the collected data.Results 1.The differences in age,serum CA125,NT-proBNP and CRP levels(P<0.05)between the observation group and the control group were statistically significant.The levels of age,serum CRP,NT-proBNP and CA125 in the experimental group were higher than those in the control group.There was no significant difference in gender,smoking history,Pa O2,Pa CO2,FEV1% pred,and FEV1/FVC% between the two groups(P>0.05);2.A comparison between the three groups of COPD patients with different degrees of PH showed that Pa CO2,serum CRP,NT-proBNP and CA125 levels had statistical differences(P<0.05).However,there was no statistical difference in gender,smoking history,age,Pa O2,FEV1% pred and FEV1/FVC% in different degrees of PH in COPD patients(P>0.05);3.Logistic regression analysis the effects of age,serum CA125,NTproBNP and CRP levels on the presence or absence of combined PH in COPD patients.The analysis results showed that serum NT-proBNP level was a risk factor for COPD patients with PH.The effects of Pa CO2,serum CA125,NT-proBNP and CRP levels in different degrees of PH in COPD patients were analyzed by Logistic regression.The analysis results showed that the serum NT-proBNP level was a risk factor for COPD patients with PH severity;4.ROC curve was used to analyze the serum levels of CA125,NT-proBNP and CRP to evaluate whether COPD patients had pulmonary hypertension,and the AUC were 0.688,0.994 and 0.755 respectively;The sensitivity was 42.9%,98.6% and 47.1% respectively;The specificity was 98.3%,96.7%and 96.7% respectively;And the yoden index were 0.412,0.953 and 0.438 respectively.Conclusion 1.Age,serum CRP,NT-proBNP and CA125 levels were correlated with combined PH of COPD patients.The levels of Pa CO2,serum CA125,NTproBNP and CRP were correlated with the severity of combined PH in COPD patients.Serum NT-proBNP was a risk factor for COPD patients with PH and its severity;2.Serum NT-proBNP level has a high diagnostic value for COPD patients with pulmonary hypertension,and serum CA125 level as a correlation indicator,poor sensitivity,but its specificity is high.
