Objective:Explore the serum markers N-terminal brain natriuretic peptide precursor(NT-Pro BNP),D-dimer(DD,D-dimer),and C-reactive protein(CRP),mean platelet volume(MPV)levels and blood gas analysis in the acute exacerbation of chronic obstructive pulmonary disease(AECOPD)with pulmonary artery hypertension(PH)expression and significance.Methods: 150 patients with AECOPD+PH who were hospitalized in the First Affiliated Hospital of Xinjiang Medical University from June2018 to January 2021 were selected as the research group,and pulmonary arterial systolic pressure(PASP)was measured by echocardiography According to the level of PASA,the150 subjects in the study group were divided into three groups: light,medium,and heavy.The 100 AECOPD patients treated in the hospital during the same period served as the control group.The basic data,NT-Pro BNP,D-D,CRP,MPV,blood gas analysis and cardiac ultrasound results of the above population were recorded.Results: The NT-Pro BNP,DD,CRP,MPV,and arterial partial pressure of carbon dioxide Pa CO2 in the AECOPD+PH group were all higher than those in the AECOPD group(P<0.05).The arterial oxygen partial pressure in the AECOPD+PH group Pa O2 was lower than the AECOPD group(P<0.05).There is no difference in the indicators of DD,CRP,MPV,arterial partial pressure of carbon dioxide Pa CO2 and arterial partial pressure of oxygen Pa O2 in the light,moderate,and severe groups of AECOPD+PH patients There was no statistical significance(P>0.05).There was a statistically significant difference in NT-Pro BNP between the mild,moderate,and severe groups.Further pairwise comparisons showed that the NT-Pro BNP level of the severe group was higher than that of the moderate and mild groups.The NT-Pro BNP of the group was higher than that of the mild group,and the differences were statistically significant(P<0.05).The person correlation analysis shows that there is no correlation between PASP and MPV,Pa CO2 and Pa O2 indicators,and the difference is not statistically significant;Spearman correlation analysis shows that there is a relationship between PASP and NT-Pro BNP indicators Positive correlation,the difference is statistically significant,there is no correlation with DD,CRP indicators,the difference is not statistically significant.The results of multivariate logistic regression analysis showed that NT-Pro BNP,Pa CO2 and Pa O2 have an impact on the occurrence of PH,and their independent influencing factors,that is,patients with high NT-Pro BNP,high Pa CO2,and low Pa O2 are likely to have PH,And DD,MPV and CRP indicators have no effect on the occurrence of complications,and the difference is not statistically significant.ROC curve analysis shows that NT-Pro BNP,Pa CO2 and Pa O2 indicators have predictive power for the occurrence of AECOPD patients with PH,and the greater the value of NT-Pro BNP,Pa CO2,the stronger the predictive power,And Pa O2 the smaller the index value,the stronger the predictive ability.Conclusion:(1)NT-Pro BNP,D-D,CRP,MPV,hypoxia are involved in the formation of AECOPD and PH.(2)NT-Pro BNP is correlated with PASA,and the degree of PASA can be preliminarily estimated.(3)NT-Pro BNP,Pa CO2 and Pa O2 indicators have predictive power for the occurrence of PH in AECOPD patients.
