Objective:The aim of the study was to observe the changes of serum cystatin C(Cys C)in acute exacerbation of chronic obstructive pulmonary disease(AECOPD)patients and to analysis the relationship between serum Cys C levels and pulmonary function,serum CRP levels and pulmonary artery systolic pressure(PASP).Methods:Serum cystatin C level was determined in groups of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)(n = 100)from January 2016 to January 2018.The general clinical datas,pulmonary function and serum Cys C levels were compared among no-PH group,little-PH group and moderate-severe-PH group,and the correlations between serum Cys C and CRP level or pulmonary artery pressure were analyzed.Results:There was no significant difference in smoking(yes/no),the sex ratio(male/female)and BMI among three groups of AECOPD patients(P>0.05),but age was different and statistically significant(P < 0.05).Among three groups of AECOPD patients,there were significant difference and statistically significant in FEV1/pre?FEV1/FVC?RV/TLC?DLCO?Cys C?CRP and PASP were different(P < 0.05).The serum Cys C was positively correlated with RV/TLC(r= 0.392,P<0.001),serum CRP(r= 0.519,P<0.001)and PASP levels(r= 0.413,P<0.001),The serum Cys C was negatively correlated with FEV1/ predicted %(r=-0.387,P < 0.001),FEV1/FVC(r=-0.433,P<0.001)and DLCO(r=-0.297,P<0.001).Conclusion:Serum Cys-C level is correlated with the elevation of inflammatory biomarker,the elevation of PASP and the decline of lung function in AECOPD patients,which could be used as an evaluation indicator for the evaluation of AECOPD patients.Monitoring Serum Cys-C has clinical significance for assessment of the severity of COPD patients with pulmonary hypertension.
