Objective: To understand the status of HPV infection and distribution of gene subtypes in female of Qingyang,Gansu province preliminarily,and to explore the overall situation and influencing factors of the prognosis and quality of life of cervical cancer patients in rural areas.So as to provide scientific basis for the formulation of prevention and control strategies and intervention measures for cervical cancer in this region and the improvement of patients' quality of life.Methods:(1)The HPV typing test results of 9359 women who asked for outpatient service and underwent screening for "two cancers" in a hospital of Qingyang,Gansu province from June 2016 to April 2017 were statistically analyzed to describe the HPV infection status,infection pattern and age characteristics.(2)Selected randomly 252 patients from January 2014 to March 2017 in reimbursement information of the New Rural Cooperative Medical Insurance system who were diagnosed with cervical cancer as investigation object.Choose the self-made questionnaire,the core cancer patients quality of life scale and cervical cancer specificity scale with Chinese version from European organization for research and treatment of cancer as survey tools.For nearly four weeks had sex of 43 patients,the female sexual function index scale questionnaire with Chinese version was conducted.Analyzed statistically scale score,combined with the general situation and diagnosis and treatment information to carry out single factor and multiple linear regression analysis,to find out the relevant factors affecting the quality of life of patients.Results:(1)The positive rate of HPV infection was 25.81%,the rates of high-risk,low-risk and multiple infection were 18.69%,4.22% and 2.91%,respectively.The most common subtypes of overall infection were HPV16,58,54,52,51,and 53,infections of high-risk accounting for 51.17% of total infections.(2)The overall infection rate was the highest in the great than or equal to 65 age group,and the high-risk infection rate was the highest in the group of from55 to 64 age.There were some differences in the distribution of subtypes among age groups,but HPV16 and 58 were distributed in all age.(3)HPV infection in women of different age is mainly single infection pattern,and mainly high-risk single infection.(4)The study showed that the global health score of quality of life was 49.34±23.61.(5)The results of EORTC QLQ-C30 indicated that the lowest score was 66.38±18.09 for social function in the functional dimensions,and the highest score was 66.80±33.47 for economic hardship in the symptom dimensions.(6)The EORTC QLQ-CX24 score showed that peripheral nerves,body image and lymphedema were the most prominent problems in the respondents,and the scores were 37.70±31.82,35.10±27.75 and33.02±27.51 respectively.(7)Only 17.06% of the respondents had sex last four weeks,and the overall score of sexual function was 20.40±4.72.(8)The results of multiple linear regression analysis of the respondents' quality of life showed that the five factors,hospital level of diagnosis,economic status,ability of emotional regulation,husband's occupation and working intensity were related to the global health status of the respondents.Conclusion: HPV infection in Qingyang female located a higher level,and there were certain regional characteristics in age and type distribution.The prognosis of patients with cervical cancer in rural areas was low quality of life,and the prognosis of patients with poor sexual function.The factors influencing the quality of life of cervical cancer patients are interact comprehensively in various aspects.The higher level of diagnosed hospital,the better family economic situation,the better ability of emotional regulation,husband go out as migrant worker and moderate work intensity,there were better quality of life.It is suggested that medical institutions should improve the level of diagnosis and choose personalized treatment programs for cervical cancer.Nurses should provide timely psychological counseling and sex guidance.Families should give more care to help them return to normal work and life as soon as possible.
