ObjectiveTo construct a PERMA intervention model suitable for Chinese cultural characteristics and the condition of patients with early cervical cancer,so as to provide reference for clinical nursing intervention.Methods1.Using qualitative research method to understand the current situation of sexual relationship experience of patients with cervical cancer after radical hysterectomy.10 postoperative patients with cervical cancer who were followed up in the outpatient department were recruited in three tertiary hospitals using purposive sampling method and a semi-structured interview was conducted with them.Following Colaizzi's method of analysis, data were analyzed with Nvivo12.0 software.2.Through the literature analysis, and combined with the current situation of sexual relationship experience and psychological characteristics of postoperative patients with cervical cancer, the content framework of PERMA intervention model was proposed preliminarily. To furtherly revised the intervention model by expert meetings and pre-experiment.3.A randomized controlled trial was conducted to verify the effect of the PERMA intervention model.A total of 91 postoperative patients with cervical cancer who were followed up in the gynecological outpatient department of three tertiary hospitals in Chongqing from November 2018to June 2019 were sampled by convenient sampling, and assigned into the experimental group and control group according to random number table, with 46 cases in the experimental group and 45 cases in the control group. The control group received routine health education, and the experimental group received PERMA intervention on the basis of the control group. The Female Sexual Function Index scale,Self-rating Depression Scale and Index of Well-being scale were used to evaluate baseline and evaluated the effects of the intervention after 3 and 6 months.Results1.Qualitative research:The sexual and intimate relationship experience of postoperative patients with cervical cancer was poor,it could be classified into four themes:sexual distress, female role trauma, changes in marital relationships, changes in self-perception.There was a certain relevance between the themes.2.PERMA intervention model was constructed:(1)The theoretical basis of the model is PERMA happiness theory of positive psychology;(2)The team was led by nurses and cooperated with multi-disciplinary experts;(3)The operation mode were mainly in the form of WeChat consultation and guidance, public number knowledge publicity and education, network video teaching;(4)The implementation process were four stages:P plan–maintaining positive emotions(sexual psychological of rehabilitation guidance);E plan–experience a sense of immersion(sexual physiological rehabilitation guidance);R plan–building harmonious relationships between the sexes(guidance of sexual skills);M-A plan–set goals and experience a sense of accomplishment(comprehensive guidance).3.Randomized controlled trial to verify the effect of PERMA intervention model:(1)Before the intervention, there were no statistically significant differences in general information, scores of Female Sexual Function Index, the number of cases in each degree of depression degree(no,mild, moderate, severe)and Index of Well-being(low,moderate,high)between the two groups(P>0.05);(2)After intervention, scores of Female Sexual Function Index, the number of improved cases in each degree of depression degree and Index of Well-being in the experimental group were higher than that in the control group, and the differences were statistically significant(P<0.05);(3)Pearson correlation analysis results showed that Female Sexual Function Index was negatively correlated with depression degree(r=-0.612,P<0.001),but it was positively correlated with Index of Well-being(r=0.638,P<0.001).ConclusionThe sexual relationship experience of patients with early-stage cervical cancer after radical hysterectomy was poor, which seriously affected patients living quality. The PERMA intervention model constructed in this study can effectively improve the sexual life quality of patients with early-stage cervical cancer after radical hysterectomy, alleviate their psychological depression and improve their subjective well-being.The model deserves further research and promotion, and we could explore synchronous intervention programs, which is suitable for patient's partner.
