Objective:To understand the status of patient delay of oral cancer patients;And I explored the influence of general information on patient delay,health literacy and coping style;I analyzed the correlation among patient delay,health literacy and coping style of oral cancer patients;I explored the mediating effect of coping style on patient delay time and health literacy,so as to improve the health literacy of patients with oral cancer,reduce the phenomenon of patient delay,and improve the quality of life.Methods:A questionnaire survey was conducted among 170 oral cancer patients in the Department of Oral and Maxillofacial Surgery of the Hospital of Stomatology of the University of Jilin,Jilin Province from November 2018 to October 2019.The survey tools included general information questionnaire,medical seeking situation questionnaire,Health Literacy Management Scale and The Medical Coping Modes Questionnaire.The database was established by software Epidata 3.1.The data were analyzed by the IBM SPSS Statistics 22.0 software.In the study descriptive statistical analysis was used for the general socio-demographic characteristics of the patients,statistical methods such as t-test and one-way ANOVA were used for the univariate analysis,and the unconditional Logistic regression analysis was used to analyze the relationship between influencing factors of medical treatment delay in patients with oral cancer,multivariate stepwise regression analysis was used to analyze the coping style and health literacy of oral cancer patients,correlation analysis of patients’ medical treatment delay,coping style and health literacy was conducted by Pearson correlation analysis and Spearman correlation analysis.The mediation effect of data was analyzed using Amos 24.0 software.Results:1.In this study,170 questionnaires were issued,167 questionnaires were recovered,the recovery rate was 98.24%,158 questionnaires were valid,the effective recovery rate was 92.94%.2.The elapsed time of oral cancer patient from discovery to the seeking for medical advice for the first time ranged from 2 days to 4 years,the median was 36.5 days;There were 100 patients with medical delay and 58 patients without medical delay,the incidence of oral cancer patient delay was 63.3%.3.Oral cancer patient delay single factor analysis showed that age,education level,monthly average income,working status,smoking status,medical payment status,consciously disease severity,permanent residence,living conditions,regular physical examination and ways to find oral problems was statistically significant(P < 0.05).Binary classification unconditioned Logistic multifactor analysis illustrated that has a significant impact on patient delay variables(P < 0.05)are age,smoking status,consciously disease severity,regular physical examination.4.The coping style scores of patients with oral cancer were(42.20±4.21).the score of confrontation coping style was lower than the norm of domestic patients,the score of avoidance coping style was higher than the norm of domestic patients(P<0.05),and there was no significant difference between the score of resignation coping style and the norm of domestic patients.5.The health literacy score of oral cancer patients was(64.70±7.85).There were significant differences in sex,age,education level,working status,permanent residence and regular physical examination(P < 0.05).6.Patient delay was negatively correlated with health literacy and its various dimensions,and coping style,and health literacy were positively correlated with coping style(P < 0.01).7.The direct effect of health literacy on the patient delay time was-0.292,coping styles play a part of mediating effect between patient delay time and health literacy,and the mediating effect value was-0.407.The total effect was-0.699.Conclusions:1.Patient delay was common in patients with oral cancer and needed to be further improved,with 63.3% of patients suffering from delayed medical treatment,and age,smoking status,consciously disease severity,regular physical examination are the main factors influencing the oral cancer delay.2.Patients have low health literacy,and gender,age,education level,working status,permanent residence,regular physical examination are the main factors influencing it.3.The coping styles adopted by patients were confrontation,avoidance and resignation.The results of multiple factors showed that age,education level,monthly average income,living conditions were the influencing factors.4.There was a negative correlation between patient delay and health literacy and coping style in patients with oral cancer.The higher the level of patients’ health literacy,the more likely they are to respond positively,and the less likely they are to patient delay.5.Coping style plays a part in mediating between patient delay time and health literacy.The higher the level of health literacy of patients with oral cancer,the more likely they are to adopt confrontation coping style,actively cooperate with support and help from family and friends,and the lower the incidence of patient delay.In order to reduce the phenomenon of patient delay of oral cancer patients,measures can be taken to strengthen the public’s knowledge of oral cancer,improve the level of social support and improve the medical insurance system,so as to seek early detection and treatment. |
