Research Of Status For Patient Delay With Oral Cancer Patients And Its Influence Factors

Objectives: Pack and Gallo first suggested patient delay this concept in 1938,and defined as the first time to see a doctor when the patient find the body unwell in this stage,this period of time more than three months as the standard definition of the patient delay.Oral cancer at the top of the whole body malignant tumor of the sixth,although in recent years for the treatment of tumor has made great improve,but the 5-year survival rate of oral cancer patients is still not optimistic,mainly related to disease stage,after the healing in early stage after the effect is better.Medical screening on a regular basis is helpful to early diagnosis of cancer,due to our country is a populous country,economic development is not high,level of medical and health development is lacking,hard to satisfy the crowd in cancer screening work,by shortening patient delay helps to improve the early detection,early diagnosis and early treatment ",it is of positive significance for oral cancer patients after operation.Therefore,it is necessary for us to survey the patient delay in patients with oral cancer and its influencing factors.This study through the questionnaire survey to our hospital patients with oral cancer patient delay,through data analysis,discuss the influence factors of delay,and in order to provide reference basis for the prevention of oral cancer.Methods: The researchers apply to the competent department of the hospital,without obtaining permission from the hospital,department leadership,selected from January 2015 to August in xx hospital questionnaire survey to 72 oral cancer patients.By face-to-face interview method to investigate,premise is that under the condition of patients’ informed consent and good mental state,the researchers adopt unified guidance language tell patients,specific requirements,and fill out the purpose of this study method,in secret way,in every patient’s survey,part of the questionnaire conducted by doctors in hospitalized patients with initial evaluation,while the rest of the questionnaire by patients families independence and(or)with truthfully fill in according to actual condition,to recover on the spot,and check the questionnaire,in order to ensure the validity of the questionnaire.Then use SPSS17.0 software for statistical data analysis,Oral cancer patients using rank and inspection to compare different characteristics of the differences in time to go to a doctor,and using the correlation analysis and binary classification unconditioned Logistic multi-factors analysis to study the relationship between oral cancer patient delay and its influence factors.Result: 1.The incidence of oral cancer patient delay was 48.5%;The elapsed time from discovery to the initial seeking for a medical consultation ranged from 2 days to 5 years,the median was 120 days,quarterback spacing for the(60,345).2.Oral cancer patient delay single factor analysis showed that age,with other diseases,consciously disease severity,understanding of the knowledge of disease,degree of anxiety and family support was statistically significant(P < 0.05).Correlation analysis for medical treatment delay was correlated with the four major factors,including the disease knowledge,anxiety,depression,and family support was statistically significant.Binary classification unconditioned Logistic multifactor analysis found that has a significant influence on patient delay variables(P < 0.05)are living conditions,family support,the severity of disease,degree of anxiety.Which living conditions,family support,consciously disease severity OR value is less than 1,the degree of anxiety OR greater than 1,namely,living conditions,family support,consciously disease severity is the protection factor,anxiety is a dangerous factor.Conclusion: Through the study found that patients with oral cancer patient delay phenomenon is relatively common,48.5% of patients in patient delay(symptoms found for the first time to see a doctor for the first time in a period of 3 months or more),and living status,perceived severity of disease,anxiety,and family support are the main factors influencing the oral cancer delay,the people who live alone than social groups are more likely to happen delay,the higher the degree of anxiety,the more prone to delay,family support and consciously the higher disease severity is not easy to happen delay,in order to improve the oral cancer patients life quality and prolong the survival time,we should pay attention to medical treatment in patients with oral cancer delays the implementation of effective intervention measures,to hope can effectively reduce the incidence of oral cancer patient delay,make its can get effective treatment early in the disease.
