Research On Characters Of Syndrome Element Differences Of Different Sleep Quality In Chronic Atrophic Gastritis

ObjectiveHuangdi Neijing says stomach distress can lead to poor sleep.Clinically,a lot of chronic atrophic gastritis patients have combination symptoms of insomnia.This study will focus on clarifying the characters of syndrome elements within chronic atrophic gastritis patients with different quality of sleep and comparing the differences of syndrome elements to provide a reference for TCM treatment of CAG patients with poor sleep quality and improve their life quality.MethodsThe subject included 120 CAG cases who had clinical symptoms.Endoscope and pathological examination of all the cases met CAG diagnostic criteria.60 of the cases who didn't have insomnia belonged to non-insomnia group,the other 60 who had insomnia belonged to insomnia group,divided by whether patients met insomnia diagnostic criteria.Through questionnaire,the basic information,“four diagnosis” information and Pittsburgh sleep quality index(PSQI information)of all the cases were gathered.Patients with insomnia were divided into mild and severe groups based on their PSQI points(PSQI<15 belongs to mild insomnia group and PSQI?15 belongs to severe insomnia group).Data of “four diagnosis” information was typed into syndrome element differentiation system to find out how the distribution of syndrome element was impacted by different CAG patients' sleep quality and analysis the difference of the distribution.Results1.In this study,there were no statistical differences(P>0.05)in gender and age distribution between non-insomnia group and insomnia group or between mild insomnia group and severe insomnia group.However,patients in each group tended to be middle-aged and elderly in general.2.The distribution of the characters of syndrome elements within each group showed that in non-insomnia group the primary syndrome elements of disease location were stomach,spleen,liver and kidney;the primary syndrome elements of disease nature were qi-stagnation,dampness,yang-deficiency,qi-deficiency,sputum and yin-deficiency.In insomnia group,the primary syndrome elements of disease location were stomach,spleen,mind,heart and liver;the primary syndrome elements of disease nature were qi-stagnation,qi-deficiency,yang-deficiency,dampness,sputum.In mild insomnia group,the primary syndrome elements of disease location were stomach,spleen,mind,heart and liver;the primary syndrome elements of disease nature were qi-stagnation,qi-deficiency,yang-deficiency,dampness,sputum,blood-deficiency.In severe insomnia group,the primary syndrome elements of disease location were stomach,spleen,mind,heart,liver and kidney;the primary syndrome elements of disease nature were qi-stagnation,qi-deficiency,yang-deficiency,dampness,sputum,blood-deficiency.3.While comparing the characters of syndrome elements of each group,the ranked distribution between non-insomnia group and insomnia group had statistical differences in characters of syndrome elements of heart,mind,qi-deficiency,yang-deficiency and blood-deficiency(P<0.05);Further more,mind and blood-deficiency were both risk factors in insomnia group(P<0.05,OR>1).The ranked distribution between mild insomnia group and severe insomnia group had statistical differences in characters of syndrome elements of spleen,liver,qi-deficiency and blood-stagnation(P<0.05);Further more,they were all the risk factors in severe insomnia group(P<0.05,OR>1).Conclusion1.The CAG cases without insomnia are attributed to the dysfunction of stomach,spleen and liver.The characteristics of pathological changes are mixture of deficiency and excess.And qi-stagnation and dampness are the major pathological changes.2.The CAG cases,combined with different levels of insomnia sleep quality,are attributed to the dysfunction of stomach,spleen,mind and heart.The characteristics of pathological changes are also the mixture of deficiency and excess.And qi-stagnation and qi-deficiency are the major pathological changes.
