Objective:Epilepsy is one of the most common neurological diseases.Not only does it seriously endanger the normal life of patients,but long-term medication and clinical management also bring great burden to patients,families and society.With the development of biomedicine,epilepsy with neurobehavioral comorbidities has attracted more and more attention.Among them,epilepsy with cognitive dysfunction is the most common,but its specific pathogenesis is not yet clear.Studies have shown that orexin-A(Orexin-A)not only plays a key role in the onset of epilepsy,but also has a certain relationship with cognitive dysfunction.In this study,by detecting serum Orexin-A levels in epilepsy patients,the expression levels in epilepsy patients and the relationship with epilepsy patients' cognitive function were clarified,which provided a basis for early assessment,timely intervention and improved prognosis of epilepsy patients' cognitive function.Method:In this study,67 patients with epilepsy who were hospitalized in the First Affiliated Hospital of Xi'an Medical College from December 1st,2018 to December 01 st,2019 and 53 patients with gender and age-matched health examinations who were examined at the health examination center of the hospital during the same period were selected.Collect socio-demographic data of all subjects and clinical information of epilepsy patients.The patients with epilepsy and the control group were evaluated by MMSE scale.Serum samples of epilepsy patients who were awake from the morning of admission and the fasting serum samples of healthy medical examinees were collected.The level of serum Orexin-A in each sample was detected by enzyme-linked immunosorbent assay(ELISA).Statistical analysis was performed using t test,chi-square test,one-way analysis of variance,Mann-Whitney U test,Kruskal-Wallis H test,Spearman correlation analysis,and multiple linear regression analysis.Results:1.The cognitive function of the epilepsy group decreased significantly compared with the control group(Z=-4.737,P=0.001);there was a difference in the incidence of cognitive dysfunction among epilepsy patients with different age of onset,course of disease,education level,and medication type(P< 0.05),but there is no significant difference in cognitive function among people with different age,gender,seizure type,seizure frequency,and seizure duration(P>0.05);the younger the age of onset,the longer the course,the lower the education level,the medication The more types,the lower the MMSE score(P<0.05),but there is no significant difference in the MMSE score of epilepsy patients with different age,gender,seizure type,seizure frequency,and seizure duration(P>0.05);MMSE score is positive with education Correlation(r=0.348,P=0.004),MMSE score was negatively correlated with the course of disease(r=-0.314,P=0.01),medication type(r=-0.296,P=0.015);MMSE score was related to age,gender,and There was no correlation between age of onset,type of attack,frequency of attack,and duration of attack(P>0.05).2.Serum Orexin-A level in epilepsy group was significantly higher than that in control group(t=12.542,P=0.000);serum Orexin-A level in epilepsy patients with different seizure types was different(P<0.05),age,gender,age of onset,course of disease,Education level,frequency of seizures,duration of seizures,no significant difference in serum Orexin-A levels among different types of medication(P>0.05);serum Orexin-A levels were positively correlated with seizure types(r=0.292,P=0.016),No correlation with age,gender,age of onset,course of disease,education level,frequency of seizures,duration,medication type(P>0.05);serum Orexin-A level in cognitive dysfunction group was significantly lower than that in normal cognitive function group(P<0.05),and the serum Orexin-A level was positively correlated with the MMSE score of patients with epilepsy(r=0.397,P=0.001).Conclusion:1.Cognitive dysfunction generally exists in patients with epilepsy,and is affected by many factors such as age,course of disease,education level,and type of medication.2.Serum Orexin-A levels are generally low in patients with epilepsy,and there are differences in serum Orexin-A levels of different seizure types.3.Serum Orexin-A level may become a biological marker for evaluating epilepsy with cognitive dysfunction.
