ObjectivesTo investigate the psychological status,sleep quality and its influencing factors of the second child pregnant women,and to provide scientific basis for improving the psychological status and sleep quality of the second child pregnant women.MethodsIn this study,purposive sampling was used and 162 second child pregnant women who met the inclusion and exclusion criteria were selected from January to August,2018 in an obstetric outpatient clinic or ward of a third-class A hospital in Changsha City.The general demographic data and pregnancy-related information of the second child pregnant women were analyzed with the self-designed questionnaire.The Pittsburgh Sleep Quality Index(PSQI),Self-rating Anxiety Scale(SAS)and Self-rating Depression Scale(SDS)were used to evaluate the sleep quality,anxiety and depression of the second child pregnant women.Multivariate logistic regression analysis was used to explore the influencing factors of the mental status and sleep quality of the second child pregnant women.ResultsIn this study,61(37.65%)(61/162)of 162 second child pregnant women had sleep quality problems(PSQI score>7).Among the seven dimensions of the Sleep Quality Index,subjective sleep quality,daytime dysfunction and sleeping time were the highest.Subjective sleep quality(>1)accounted for 82.72%,daytime dysfunction(>1)for 79.0%,sleeping time(>1)for 69.14%,and hypnotic drug problem detection rate was 0..The PSQI scores and total scores of second child pregnant women(except for hypnotics)were higher than the domestic norm,and the difference was statistically significant(P?0.05).Sleep quality was taken as a dependent variable and sociological data of different pregnant women as an independent variable.Univariate analysis showed that there were significant differences in sleep quality among pregnant women of different ages,educational level,family monthly income,husband-wife relationship,fetal gender expectation,planned pregnancy and gestational age score.Meaning(P?0.05).Sleep quality was taken as a dependent variable,and seven statistically significant factors,including age,education level,monthly family income,marital relationship,fetal sex expectation,whether or not to plan pregnancy and gestational age,.were taken as independent variables in univariate analysis.Logistic regression analysis was performed with forward stepwise regression(?in=0.05,?out=0.10).The final variables into the regression model were educational level,family monthly income,fetal gender expectation,marital relationship,that is,low educational level,low family monthly income,fetal gender expectation,bad relationship between husband and wife.The SAS score of the second child pregnant women was(39.42±7.18).Taking anxiety of second child pregnant women as dependent variable and different demographic and sociological data as independent variable,the univariate analysis showed that anxiety of second child pregnant women varied with age,educational level,family monthly income,husband-wife relationship,family support(parents),fetal gender expectation and whether or not to plan pregnancy.Academic significance(P?0.05).Anxiety of second child pregnant women was taken as dependent variable,and 7 factors,including age,education level,monthly family income,marital relationship,family support(parents),fetal sex expectation,and whether or not to plan pregnancy,were used as independent variables in univariate analysis.Logistic regression analysis was performed with forward stepwise regression(?in=0.05,aout=0.10).The final variables into the regression model were age,family monthly income,educational level,fetal gender expectation,whether or not to plan pregnancy,that is,young age,low educational level,low family monthly income,with fetal gender expectation.These four factors were the main factors of anxiety in second child pregnant women.The score of SDS scale of second child pregnant women was(38.59±9.61),which was significantly higher than that of domestic norm(P?0.05).There were 16 cases(9.88%)with depression,including 11 cases(13.58%)with mild depression and 5 cases(4.94%)with moderate depression.Depression of second child pregnant women was taken as a dependent variable,and different demographic and sociological data of second child pregnant women as an independent variable.Univariate analysis showed that depression of second child pregnant women had significant differences in scores of different ages,educational level,family monthly income,husband-wife relationship,planning pregnancy and abortion history(P?0.05).Depressive mood of second-born pregnant women was taken as dependent variable,and 6 factors,including age,education level,family monthly income,marital relationship,whether or not to plan pregnancy and abortion history,were taken as independent variables in univariate analysis.Logistic regression analysis was performed and forward stepwise regression method was used(?in=0.05,?out=0.10).The final variables into the regression model were educational level,family monthly income,marital relationship,that is,low educational level,low family monthly income,poor relationship between husband and wife.Conclusions1.Second child pregnant women have different levels of anxiety and depression.In the scores of SAS and SDS,the scores and total scores of second child pregnant women were higher than that of the domestic norm.19.14%of the second child pregnant women had anxiety,among which the mild anxiety was 13.58,the moderate anxiety was 4.94 and the severe anxiety was 0.62;9.88%of the pregnant women with second child pregnant women had depression.Mild depression accounted for 13.58%and moderate depression 4.94%.2.The total sleep quality of second child pregnant women was poor,and 37.65%of the second child pregnant women had sleep quality problems.The PSQI scores and total scores of second child pregnant women(except hypnotic drugs)were higher than those of the domestic norm.3.The main factors of sleep disorder in second child pregnant women are low educational level,fetal sex expectation,bad relationship between husband and wife and unplanned pregnancy.The main factors of anxiety in second child pregnant women are low age,low educational level,fetal sex expectation and unplanned pregnancy.The main factors of depression in second-born pregnant women are low educational level,and the main factors of anxiety in second child pregnant women are low educational level.The relationship between husband and wife is bad and unplanned pregnancy.
