Objective:To observe the clinical effect of acupuncture on the prevention and treatment of gastrointestinal dysfunction after general anesthesia,analyze the effect of acupuncture on the level of 5-hydroxytryptamine in patients with gastrointestinal dysfunction after surgery,and explore the mechanism of acupuncture.Methods:82 patients who met the criteria were included and divided into acupuncture group and drug group by random method,41 patients in each group.Both groups received the same preoperative preparation,anesthesia,and routine postoperative treatment.The drug group was given granisetron hydrochloride injection 30 minutes after surgery,and the acupuncture group was given acupuncture intervention 30 minutes after surgery.The bloating score,nausea and vomiting grade,and the number of bowel sounds of patients in 0-2h?2-6h?6-12h?12-24h?24-48 h five different time periods within 48 hours after surgery were assessed,the time of the first exhaust and defecation was recorded,And the serum 5-hydroxytryptamine levels of the two groups of subjects were measured before operation,30 minutes after operation,and 24 hours after operation.The data was processed and analyzed with SPSS25.0 software.Result:(1)Baseline comparison of gender,age,height,weight,body mass index,operation type and operation time between the two groups showed no statistically significant differences(P>0.05),which was comparable.(2)The scores of abdominal distension,nausea and vomiting in the acupuncture group were lower than those in the drug group at the three time periods of 2-6 hours,6-12 hours and 12-24 hours after operation,which difference was statistically significant(P<0.05).The number of bowel sounds in the acupuncture group was higher than that in the drug group at 2-6 hours,6-12 hours,12-24 hours and 24-48 hours after operation,which difference was statistically significant(P<0.05).(3)The time of first postoperative exhaust and defecation in the acupuncture group was earlier than that in the drug group,which difference was statistically significant(P<0.05).(4)Comparison of 5-HT within the group: The 5-HT concentration at 30 minutes after surgery in acupuncture group was lower than before surgery,and the difference was statistically significant(P<0.05);The 5-HT concentration in the acupuncture group at 24 hours after operation was higher than that at 30 minutes after operation,but the difference was not statistically significant(P>0.05);There was no statistically significant difference between the acupuncture group before operation and 24 hours after operation(P>0.05).The 5-HT concentration at 30 minutes after surgery in drug group was lower than before surgery,and the difference was statistically significant(P<0.05);The 5-HT concentration in the drug group at 24 hours after operation was lower than that at 30 minutes after operation,but the difference was not statistically significant(P>0.05);The concentration of 5-HT at 24 hours after the operation decreased significantly compared with that before the operation,and the difference was statistically significant(P<0.05).(5)Comparison of 5-HT between the groups: There was no statistically significant difference between the two groups before operation(P>0.05),suggesting comparability.There was no statistically significant difference between the two groups 30 minutes after operation(P>0.05),The concentration of 5-HT in the acupuncture group was higher than that in the drug group at 24 hours after operation,and the difference was statistically significant(P<0.05).(6)Safety evaluation: None of the subjects experienced safety problems.Conclusion:(1)Acupuncture can prevent or reduce postoperative abdominal distension,nausea and vomiting and other clinical symptoms.The overall effect is better than granisetron hydrochloride and there are no related drug side effects.(2)Acupuncture can enhance gastrointestinal peristalsis,speed up the recovery of bowel sounds,shorten the time of first voluntary exhaust and defecation after surgery.(3)5-hydroxytryptamine plays an important role in the occurrence and development of gastrointestinal dysfunction after general anesthesia.Acupuncture is conducive to the rapid recovery of patients with gastrointestinal dysfunction after general anesthesia.However,the effect of acupuncture on 5-hydroxytryptamine still needs to be further studied on different receptors.
