Objectives To explore the status quo and influencing factors of active aging,self-neglect and disability degree of the elderly.To clarify the relationship between active aging,self-neglect and disability degree of the elderly,to provide a basis for effectively delaying the disability process of the elderly.Methods The elderly in Tangshan city were studied by stratified random cluster sampling.Firstly,all 6 tertiary hospitals in the urban area of Tangshan city,Hebei province were selected,and a tertiary hospital,namely Tangshan gongren hospital,was randomly selected by the digital lottery method.Then,two community health service centers were selected from 10 community health service centers directly affiliated to Tangshan gongren hospital.From October 2018 to April 2019,a questionnaire survey was conducted on 1,105 elderly people who met the inclusion and exclusion criteria in the selected community under the administration of Tuan jie lou health service center and Fu le yuan health service center.Face-to-face interviews were conducted with participants using the General Situation Questionnaire,the ADL Scale,the Active Ageing Scale,the Elderly Self-neglect Questionnaire,the Housebound Scale,the Geriatric Depression Scale-15,the Social Support Rating Scale,and the Tilburg Frailty Indicator.The collected data were analyzed by SPSS22.0 statistical software package,including descriptive analysis,T test,One-way analysis of variance,multiple linear regression analysis,Pearson correlation analysis and Wen Zhonglin's mediation effect test program.Results 1 The total score of active aging of elderly was(103.81±18.27),which is at the medium level.Elderly active aging level of multivariate linear regression analysis showed that age,education level,religious beliefs,hobbies,number of falls,smoking,drinking,Housebound,weakness,monthly income,financial resources,medical burden,depression and social support are independent factors influencing the active aging of the elderly.2 The total self-neglect score of the elderly was(9.26±4.15),which is at a mild to moderate level.A multiple linear regression analysis of self-neglect among older adults showed that older age,lower literacy,higher falls,smoking,Housebound and weakened status,low income,economic source is not pension,heavy medical burden,depression,no children and low levels of social support were the reasons for higher self-neglect among older adults.3 The incidence of disability among the elderly was 33.6%,including 208(19.4%)elderly with mild disability and 153(14.2%)elderly with severe disability.A multiple linear regression analysis showed that disability in the elderly is affected by gender,age,number of falls,number of chronic diseases,home stay,decline,monthly income,financial resources and medical burden.4 Pearson correlation analysis results showed that active aging in the elderly is negatively correlated with disability(r=-0.618,P<0.05),and self-neglect in the elderly is positively correlated with disability(r=0.614,P<0.05).5.Mediating Effect the results showed that there is a partial mediating Effect between the degree of active aging and the degree of disability in elderly self-neglect(P<0.05),the mediating Effect is-0.260,the total Effect is-0.618,and the contribution rate of mediating Effect to the total Effect is 42.0%.Conclusions 1 Active aging is negatively correlated with disability of the elderly,while self-neglect is positively correlated with disability of the elderly.Self-neglect mediates the effect of active aging on disability in the elderly.2 Active aging in the medium level in the elderly,young,high-cultural level,religious beliefs,interests,no fall,no smoking,no drinking,no in Housebound and weak condition,high income,economic source is pension,medical treatment burden,depression and high level of social support is the protection of the elderly active aging factors.3 Older adults,those with lower educational attainment,higher falls,smoking,Housebound and weakened conditions,low monthly income,non-retirement income,heavy healthcare burdens,depression,childlessness and low levels of social support had higher levels of self-neglect.4 The incidence of disability among the elderly was 33.6%.Women,advanced age,high number of falls,high number of chronic diseases,home stay,weakness,low monthly income,non-pension income and heavy medical burden are risk factors for disability in the elderly.Figure1;Table23;Reference 168.
