Objective Periodontitis is one of the major causes of tooth loss,which affects people's diet and quality of life.Periodontal basic therapy is the primary and effective method to treat periodontitis.The clinical effect of periodontal local treatment before and after treatment is quite significant.With the deepening of the research,the intervention of systemic diseases is an important part of the research on periodontal basic treatment.Type2 diabetes mellitus(t2dm)is a metabolic disease caused by glucose metabolism disorder,which is often associated with periodontitis.It is known that there is an interaction between periodontitis and diabetes,but the exact cause-and-effect relationship and mechanism are unclear.The purpose of this study was to investigate the control of periodontal inflammation in patients with type 2 diabetes mellitus complicated with periodontitis after periodontal basic treatment,and to analyze the correlation between adiponectin content and blood glucose.Methods Two groups of patients with simple periodontitis and type 2 diabetes mellitus complicated with periodontitis were selected,with 38 cases in each group.Periodontal basic treatment intervention was carried out on the two groups,including supringival scaling,supringival scaling,and root leveling.Meanwhile,the oral hygiene knowledge of the patients was publicized and educated,and the buccal periodontal sites of the same maxillary first permanent molar were selected before treatment and 8 weeks after treatment,and the gingival crevicular fluid of the patients was collected by bag end method.Adopt the adoption area in the wet mouth of the cotton roll,gently place the sterilized moisture absorbing paper tip at the bottom of the periodontal bag to collect gingival crevicular fluid,then put it into a sterile EP tube,and store it at-70?.Adiponectin levels in gingival crevicular fluid were determined by enzyme-linked immunosorbent assay(ELISA).Finally,clinical periodontal indicators before and after treatment should be examined,among which periodontal clinical indicators include gingival index,probing depth,plaque index and clinical adhesion level.The periodontal size before and after treatment was recorded.In addition,for patients with type 2 diabetes mellitus with periodontitis,fasting blood of the day of gingival crevicular fluid should be collected and extracted to measure blood glucose,and 5ml venous blood should be extracted into the EDTA blood collection tube,and stored at-4?.The total RNA of whole blood should be extracted strictly according to the instructions.Then the total RNA was reversely transcribed into c DNA and cryopreserved at-20? for later use.Real-time PCR was used to detect the expression levels of adiponectin gene in peripheral blood before and after treatment.Results Through SPSS22.0 statistical software analysis after periodontal treatment,simple periodontitis patients and patients with type 2 diabetes mellitus and periodontitis group the expression of adiponectin in gingival sulcus liquid were increased(p < 0.05),of which the periodontal clinical indices of gingival index(GI),probing depth(PD),plaque index(PLI)and attachment loss(CAL)are improved,with statistical significance.Fasting blood glucose decreased in the group of type 2 diabetes patients with periodontitis(p<0.05),which was statistically significant.The changes in adiponectin expression in peripheral blood of patients with diabetes mellitus and periodontitis(p>0.05)were not statistically significant.The oral hygiene of the patients was gradually improved and the awareness of oral health was enhanced.Conclusions The content of adiponectin in gingival crevicular fluid of type 2 diabetes mellitus patients with periodontitis increased with periodontal basal therapy.Periodontal basic treatment is also an important means to control the chronic periodontal inflammation in patients with simple periodontitis and type 2 diabetes,and can improve the level of fasting blood glucose in patients with type 2 diabetes.
